It seems your child has not been seen by a child psychiatrist or a psychologist yet. I am not suggesting one more consultation, such cases are difficult manage. Mere medical help won't do.
You might consider studying the sleep wake cycle of your child. Keep a daily calender for a week or so and see what are the time periods in the day and the night when he does sleep well. Form the description you have written, it appears he does not have a good day sleep, too. But if he does have a good 3-4 hour sleep in the day time, that's good news. We can work on the sleep schedule.
At the same time, try to note the crying episodes as and when they happen. Try to find any pattern in the whole calender that you have observed for the next week or so. It might give you some insights.
Has he been prescribed any medicine? At such a young age, medicines are not generally used.

It's been a long time since I dealt with a child, but all I can recommend from what you have told us is to see a pediatrician for his/her evaluation.
After 22 months, your child should be sleeping through the night. If he is not, it may be indicative of a medical problem and needs to be evaluate.
So get him checked and that may help all of you get some sleep.
