White substance on Penis.

I am a 32 year guy. Since 2 weeks I have been noticing a white substance under the foreskin. It is very painful when I pull back the foreskin, when I try to do that, I see some cuts. It is sometimes very itchy. I and my partner maintain good hygiene of our body. Please help.

Hello Doctor,
Thanks for all the help which you provided earlier. Everything is fine now. But 2 days back I again found the wax-like white substance on the head of the penis and under the foreskin, though I wash it twice everyday with warm water. This time it felt itchy too. Today I saw few white spots on the head of penis, though there is no itchiness. Please advise. Thanks in advance.

Hello,
If the pimple like spots were present earlier also then it can be due to Fordyce spots. However if they have appeared recently then it can be due to dermatitis skin. Do not use any harsh product and stop using Lamisil after the symptoms have subsided. Wear cotton clothes and just keep the penis and foreskin clean.

Dear Doctor,
I have been using Lamisil and washing my penis with Dettol in luke warm water. Now since past few days I have been able to retract my foreskin without any difficulty and there are no cuts but there are some pimple like spots on penis. Please advise.

Do not use any harsh products, nor any soaps.
Pull back your foreskin and wash the area gently and pat dry.

Wear cotton underwear and do not wear tight clothing.
What you have is Thrush.  This can be caught from your sexual partner and also can come on after the use of antibiotics.

Do not have sex, and apply the cream very thinly.  Apply it for a few days after the symptoms have disappeared.  If your partner has itchiness, they will need to be treated too.  Otherwise you will be passing it back to each other.

Make sure your drink plenty of water and you can also up your intake of natural yogurt to replace the good bacteria.  You can also apply it to the genital area.

Candida Albicans lives on our bodies, but when the flora is disturbed it can get out of hand and cause Thrush.  Not only can you get it on your genitals, you can also get it in the mouth.

Dear  Doctor,

I have started applying Lamisil from Novartis containing 1 % terbinafine. Also I am cleaning the penis and the foreskin with dettol. I would like to ask you how long does it take cure this thing and whether it can re-occur during a period of time.

Thank you very much for your reply. I am currently using Dettol Soap for cleaning the area. And I am cleaning it 3-4 times a day. Since I live in Dubai is there any medication which you can suggest or the contents of the medication.

Hello,
I cannot confirm without examination but the symptoms that you are having are due to balanitis of the penis.It is an inflammation of the glans penis.

Oral course of antibiotics is needed. In addition, don’t use any cosmetics on the penis, instead use some topical antimicrobials like Bacitracin or Lotrimin. Also corticosteroids and topical immunosuppressants can be used but they are available under prescription. Also retract the foreskin daily and soak in warm water to clean penis and foreskin. In case the symptoms persist then consult a urologist/dermatologist for evaluation and antimicrobials and steroids prescription.
