She'll need shots and worming a few times up until 16 weeks.  After that, as Jaybay says, vaccine schedules are changing.

Your vet will advise you on the best frequency for your area.  Certain diseases are very common in certain geographical areas, and not in others.

You will, of course, be taking your puppy to the vet within the first 2-3 days of ownership.  You will get lots of helpful info there.

It all depends - on your location, the clinic you decide to use and the individual vet.  Most shots are recommended to have boosters once a year, but parvo boosters are generally given every six months.  There are varying schools of thought on vaccine frequency these days.  Many, like rabies, actually last longer than one year.  In some communities you can skip a year or two as long as the dog's blood is tested to make certain the initial shot still provides immunity.

Call around to a few different clinics for pricing information.  You might even try contacting various shelters for low-cost vaccination information.  Congrats on your new puppy, and keep her healthy!  :-)
