Guess what? I had Crystalens implants exactly two years ago, too. We likely had the same version (4.5). It is not as good as the later versions. I too paid out of pocket. I too got no near vision and my computer distance also requires readers. I had been assured that intermediate vision was ALWAYS better. Since mine was not, I found a doctor who could do an ultrasound of my eye and discovered that debris in the capsule prevented any movement of the implants. Sometimes, this can be fixed, but sadly, not in my case. If you feel like your result is as bad as mine and want more info, let me know.
Regarding the floating cloud...I had this as well. It was the result of a posterior vitreous detachment that occurred in the postoperative period. The hyaloid membrane that is normally attached to the retina can detach and sag in front of the retina and wave back and forth. This also happened to me and the optic nerve head was attached. It was so bad, I had a vitrectomy last year to get rid of it. But this is a major procedure and I would not suggest it lightly.
The crystalens is great for distance, sort of okay for intermediate and not good for reading.
The new Crystalens HD if left about -0.50 to -1.00 for distance works good for reading. ReStor works good for near. ReZoom is in my opinion old technology and I would not recommend it to any of my patients.
Get a second opinion from another surgeon that does regular and premium IOLs
