This is in a format that I am not that familiar with but this is how I would interpret the readings. The sphere for both eyes is 0.0 D which is good. But the cylinder is -2.0 and -2..25 for the right and left eye respectively. SE is the spherical equivalent of the combined sphere and cylinder (sphere plus 50% of the cylinder). This yields the SE of -1.00 in both eyes. It is like mild myopia or being near sighted. It will be affecting distance vision somewhat. As Dr. Hagan suggests it would be worthwhile to get a full eye exam with a phoropter to get an eyeglass prescription accurate enough to buy glasses with if that is what is recommended by the optometrist.

That is a prescription for glasses. It doesn't really tell you anything about vision with/without glasses, nor about the health of the eye. Likely they just used what is called an autorefractor. I do suggest you see an ophthalmologist or optometrist for a complete eye exam.
