Vitrectomy

I have Posterior Vitreous Detachment. I have a number of floaters, including a large one that is darting about wildly, driving me crazy and impairing my ability to function. My retinal surgeon wants to perform a vitrectomy, which I understand is somewhat controversial as a treatment for floaters.

I'd be grateful for an opinion about the advisability of it, and pros vs. cons. I'm aware that it will result in advancement of cataracts, which I already have, but in early stages. The surgeon would prefer to perform the cataract surgery now as well, as he predicts I'll need it within two years if we perform the vitrectomy.

I would get a second opinion. Risks are substantial and include those you mentioned plus: infection, bleeding, retinal detachment, glaucoma, inflammation, loss of eye and persistence of floaters.    Moreover there are some new laser techniques that have been described in the medical journals that would be much less drastic if you feel you have to do something.

I personally would not undergo a vitrectomy for floaters.  I had one about two years ago to correct an epiretinal membrane that was causing significant macular distortion, and although it was a complete success I will not have one performed in my other eye unless similar symptoms develop.  You are correct about the accelerated cataract development that a vitrectomy can cause by the way.
