Mystery Black Spot When I Tilt My Head

When tilting my head or when I am physically active, I see a black spot in the lower part of ny vision near my nose.  It’s opaque and I can't see right through it and it only appears whenever I tilt my head.  It’s always in the sane spot and I can make it grow darker the harder I stretch my neck as I tilt.  What could possibly be the cause of this? I have seen two ophthalmologists, both report I have 20/20 vision and no retinal detachment. I had an MRI that was normal.  No pituitary tumor.  I had a concussion 8 years ago with pretty severe post concussive syndrome after hitting the right lower part of the back of my head, not sure if this could be related.  Blood pressure is on the lower side. I’m really baffled.  I can’t find anything online other than floaters, but this isn’t a floater.  Has anyone ever heard of this?

This could be caused by a variety of factors, including a migraine aura, an eye muscle problem, or a problem with the blood vessels in your eyes. If your previous medical evaluations haven't found a cause, it would be best to try acupuncture for migraines.

If your previous medical evaluations haven't found a cause, it would be best to consult with a neurologist for further evaluation, especially given your history of a concussion and post-concussive syndrome. It may also be helpful to keep a headache diary to track any patterns or triggers.

It is not typical of any specific problem. If you live in a large city with a university I would make an appointment with a neuro-ophthalmologist and make sure you have a peripheral visual field test. If you want to push with with local eye exam see an ophthalmologist that specializes in retina and vitreous. Besides sectoral RD, the problem could be a splitting of the retina which can be very difficulty to see (retinoschesis) and also rule out old peripheral retina vein or artery occlusions
