eye trauma

Hello Dr> Well this is my story, 27 days ago i suffered from blunt trauma right on my left eye. A day after the accident i had a lot of black spots(floaters). And my vision seemed really blurry. This happened on a friday night, so i had to wait till monday because since i don't have insurance i didn't want to visit the ER. So on monday i went to see my opthamologist and he noticed that my pupil didn't react to light. He told me that i had a torn sphincter. Even before he prescribed me with cyclopentolate drops my pupil was almost the size of my iris. After about 1 1/2 weeks my pupil decreased in size somewhat. not much tho. I have kept seeing my MD. They told me that i was lucky because it seemed that the optic nerve was ok, my retina was ok aswell, and that i didn't have a dislocated lens. yeah the only problem was my pupil. My doctor told me that there was little probability of my pupil ever healing i guess. i'm still on pred-forte 4x a day because my iris is still inflamed.Before the accident my corrected vision was 20/20 now is 20/30. My near vision right after trauma was blurry, now it improved a little. Also my pressure was a little high at first now it stabilized back to normal. Ok now this is my question: I have read about Pilocarpine drops and Thimoxamine drops: What do you think about this drops? Well My MD told me we can use the drops, but she said till my iris goes back to normal, because is still inflamed.

Any points, recommendations,knowledge and wisdom  or any possible solutions that might help me will be really appreciated! Thanks Dr: JCH :)

Pilocarpine will not help an iris with a torn sphincter.  It is worth a try since you do not know the degree of damage.

Well thanks for your reply Doctor. I kind of read some information about your educational background and saw that you went to cedars-Sinai. I actually had My eye checked at this facility and i spoke with Dr. Oleynikov. He told me that there's not much hope for my pupil. I actually would like to ask you something that concerns me, before the injury i was able to distinguish objects at night or dark rooms. Now it seems that i can't see nothing in dark spaces. When i compare my 2 eyes there is an obvious difference. two MD's already checked my retina with the special round lens they use and told me that my retina seemed fine. But what can be causing the dark vision in my eye that had trauma. Is there any test that can be done to diagnose this problem. And would seeing a retinal specialist help, even tho i have already seen 2 ophthalmologists.
Thanks for responding to my first question, and i hope to find out what could be causing this problem, since i know it was caused by my blunt trauma.!!

im 25 year old male. Need help. Just got a job at a restaurant and my vision is driving me crazy.
I had a cataract removed with the acrysof IQ monofocal on  my left eye. My eye prescription before cataract surgery was -.75x-3.500x020(od)
                                                                       -4x-1.25x010     (os)

after cataract surgery                                   -4.25 +3.75x110(od) plus prescription
                                                                         +.50+.75x090    (os) plus prescription

I don't have cataracts on my right eye, so there seems to be an imbalance driving me crazy. I didn't have an imbalance before the surgery. I also have keratoconous, so prk/ lasik is not an option. Can you tell me the diopeter difference of two eyes post up cataract surgery? According to surgeon it is 2.25 which he said my eyes should be able to handle but they're not. For some reason i ended up farsighted on left eye instead of plano. Any suggestions as tho what should i do?
the surgeon optometrist suggested using a contact lens, i have always had some problems with contacts so i will have to see if they actually help. my anisenikonia is refractive, so what would you say i do now. cheapest solution since most of my savings went into the cataract surgery. any advice will be greatly appreciated it

Actually, I'm Japanese and unfortunately, I got hit in my eye 3 month before the day I was planing to go back to Japan so I guess it was bad timing. Anyway, I do plan to go to a retina specialist once I go back. The doctor doesn't know what it is, but I developed a tiny gray spot in my center of the vision, but fortunately, it seems to gradually disappearing. The doctor said I had two tiny spots on my retina one month ago, and I have my next appointment next week so I guess I have to wait and find out if my retina healed well. Also, I'm only 15 year old so it scares me when I think about what can happen to my eye in the future because of this incident. How is your vision in the dark, any better?

Have u gone to a retina specialist. I guess they're more specialized, and they'll be able to see what's going on. I'm not sure if u ever got floaters or flashes, but I guess your vision is back to normal except at night or when looking at a paper and stuff..

Hey Karp989. Last month, my left eye was injured from a racket hitting directly to my eye. So far, no retinal problem but I have experienced the same thing as you. During the first after following the injury, I couldn't see anything in the dark with my injured eye. Now after a month, I am able to see objects in the dark but I also notcied that since the injury, my left vision got darker than my right eye. It is not that noticible but when I look at a paper, the paper would like darker than my right vision. I hope that my eye is still healing and I wish you good luck to your left eye.

MEDICAL PROFESSIONAL
The two mostly likely possibilities are optic nerve damage  or retinal damage.  A careful pupil exam will help determine this.  Read up on afferent pupillary defect.
