Members of our ophthalmology group just returned from one of the most important meetings on results of "Premium IOLs". By far the best results were with Crystalens, then ReStor and last place ReZoom.

As I read your problem you have a crystalens in your eye. That is NOT a piggyback IOL which means that there are two IOLs in the eye.

The risks of yag capsulotomy are very small. There can be a temporary increase of pressure but drops are  now used to prevent this and it is a rare problem.

With the capsule open the risk of a retinal detachment in very long previously myopic eyes and/or relatively young individuals increases by .5 to 1.0%.

With a crystalens IOL the risks of yag causulotomy increase as it might move the position of the IOL or make it less effecient in focusing. Also once the posterior capsule is open the difficulty and risk of a IOL exchange goes up.

If you have confidence in your Eye MD surgeon stick with him/her as working through complications is where the skill and expertise of a surgeon is most important. If you have any doubt then obtain a qualified second opinion.

When cataract surgery is done the back wall of the lens (posterior capsule) is left in the eye to stabalize and hold the implant in postion. In the months and years after surgery that membrane can turn cloudy (like a window getting dirty). The symptoms of the cataract often return such as blurred vision, glare, poor night vision and difficulty reading and driving. This is called a secondary cataract or posterior capsular opacification.

When it becomes a problem a yag laser is used in a relatively simple procedure to cut a hole in the membrane to see through. The recovery is within 24 hrs. and usually no drops or restriction of activity is necessary. At present the percentage of cataract surgery patients that need a yag laser is about 30%, higher in younger people and in older types of implants.
