LvnEmbryos..question for you

Hi,
First of all you're NOT overstepping at all...we're all glad that you're here!  Question for you....I just did my first IVF and it failed..so sad.  My story.  I have a 3 year old that was concieved sooooo easily.  After that I've had two early miscarriages.
    Borderline FSH....I had 9 eggs...they retrieved 7....all 7 fertilized...all normal.  On day 3 my RE said that I have 4 excellent looking embryos.  I asked her to put in three but she said that these 4 looked so good that she only felt comfortable putting in 2 because they were top quality.  Well..the test was negative.  RE was surprised too because she said that she didnt have a reason for it because these eggs looked great.
  So now I'm beginning a FET...started my period today...I'll be taking estrogen tablets for the next 10 days then I go in for a lining check next Thursday..they were able to successfully freeze my remaining 5 embryos, which the nurse said was good because they only freeze good ones I guess.  They were frozen at the 8 cell stage, beginning to compact...when they thaw them it sounds like they want to try to take them to blast, and do assisted hatching.  Just wondering what you think about this protocal...and do I have much of a chance with 5 frozen??? My RE says about a 40% chance..what do you think?

Sorry it took me so long to get back to you...busy week at work!  5 is a great number don't be discouraged.  Usually only about 50% make it from fertilization to blast, so your numbers are incredible!!  We don't do thaws on day 3 but our survival rate for blasts is 80% so your numbers sounds good.  We will usually freeze anything that is fantastic but like to freeze at least 2 at blast in case one does not survive.  I can't give you a % success rate, but your odds sound good of having a successful transfer.  Without knowing more about your embryos its hard to say.

If this fails I think you should definitely try again, and go to blast.  That is our normal protocol, day transfer on first cycle and then if no pregnancy blast transfer on the 2nd cycle.  The only difference is if we do a day 3 transfer we still culture them to blast and freeze them at blast, but thats just our prefence.  And also, you cannot do assisted zona hatching on blastocysts, the embryo is too close to the zona and they really don't need the help.  A lot of blasts are already hatching on their own by transfer.  GL!!!

Yes...9 follicles....7 eggs.  All 7 fertilized and all we're "normal".  They wanted to do a day 3 because I guess that they thought that 7 wasnt enough to take to blast...do you agree?
   All of my 7 made it to day 3...do a lot usually arrest before then?  Realistically what do you think my chances are of a FET with 5?? I'm just worried because only having 5 frozen seems like a small number.
  IF this fails...from all that I posted...do you think it's worth trying this again? And what would you do different? Try to get them to blast with assisted hatching?

Ok so a few things I wanted to clear up.  You said you had 9 eggs and 7 retrieved.  You probably had 9 follicles.  There are not necessarily eggs in all follicles and some follicles will have immature eggs or the eggs will not come out with the aspiration.  But great numbers, very impressive to have all 7 fertilize normal and all 7 make it.  

Taking the embryos to blast is probably a good idea because only 50% of embryos make it to blast, so if they wait until day 5 they can pick the two best ones.  I am not aware of any clinics who do assisted zona hatching on day 5.  The trophectoderm of the embryo is SO close to the zona it is hard to break the zona without damaging the embryo.  And a lot of blasts will already be hatching by the time of transfer, just an FYI.  

40% sounds like a really good estimate.  If you had so many make it to day 3 that is a great start.  Like I said, waiting until day 5, if they thaw all 5 (which we probably would) then they can be really choosey on day 5.  I know our thaw rates are not fantastic for day 3 (we actually don't freeze on day 3) so hopefully they will account for that too because not all embryos survive freezing even in the best case scenario!

Thanks Tina...how are you doing too?? Any luck?? What are all of your options?? I just know that i cant keep doing IVF's over and over again....I really want this to work but if it doesnt I cant do infertility treatments for the rest of my life!!!
