I personally would not do this. If a doctor sees this then it can give them a preconceived idea about you. For example, if you put FMS/CFS on there many doctors will assume it's "all in your head". If I see a new doctor I want their unbiased opinion so I just give them the basics...symptoms, allergies, why I'm here. I would admit to diagnoses that are objective like high blood pressure, high cholesterol etc...
HI Patsy,
Please read under
Health Issues. DIAGNOSED this means a doctor I have a Neurologist
who DIAGNOSED my Fibromyalgia, Restless Leg, RSD.
My doctors know me, not a whining person who is in their office to waste either my time or theirs. IF I AM IN THEIR OFFICE I AM IN PAIN OR SICK.
Preconceived no, just the facts to get results.

Diagnosed or not. It does not mean the diagnosis is correct. I have had several incorrect diagnoses. Another doctor may not even believe these diagnoses exist. All three of them are controversial diagnoses. None are really provable. I won't argue about it. Just giving my opinion. I have a fibro diagnosis myself. I would never admit this to another physician. I don't even want it in my chart. I believe it is a trashcan diagnosis. Remember, fibro is "a group of symptoms". It is not a disease. Neither is restless leg or RSD. All are symptoms. I want doctor that will keep looking for the cause.
I too have been misdiagnosed .
I know this about all the disorders I have also have Narcolepsy, 17 years.
They are called idiopathic (unknown).
I think you should look much as you want.
I am never opposed to anyone's opinion.
None of us know it all, I am not a doctor, just trying to help someone who may not know
how to communicate their feelings to get some form of help.
Isn't that what this site is for to help each other?
bye

I think it depends on the individual dr. I know a lot of dr's who would appreciate this type of update. A clear, consice statement of what brought you to the dr's office that date. I have taken one several times to my PCP, to update him on what specialists are treating, symptoms, etc. He always puts it in my chart. In the extreme case of filing for a disability claim, it substantiates long term problems you are having.
THIS HELPS THE YOUR DOCTOR TREAT YOUR INDIVIDUAL HEALTH ISSUES.
38 ALLERGIC REACTIONS:: Some are called Opposition Responses like to SSRIs,
SSNRIs (serotonin), Lyrica, Tramadol, Codeine, they actually make me hurt more than the pain they are suppose to treat also give me panic attacks. Rashes, anaphylaxis, swelling, joint pains.
Thank you, have read several of your helpful comments to others good job.
