Gut/Pelvic/Testicular pain. Where should I start?

History:
I am around 350 Lbs, I smoke ciggarettes and pot, I eat a ton of fiber, strictly consume <1900c a day, and drink too much sugar via soda pop. 1 Grandmother died of colon cancer, the other has osteoperosis both grandfathers are healthy last I checked, and in their 80s. Father has sleep apnea, High blood pressure. Mother has R. Arthritis,

Background:
About a year ago I had difficulty passing a bm. this lasted for about 15 days before I went and saw a doctor.  Doc told me to give myself an enema, and I did. I never passed a very large stool as he told me to be prepared for.

Fast forward a year, and a multitude of new symptoms have shown up. I have made a med - outline dude - diagram of my symptoms. I find describing these things without being able to point at it kinda non specific.

Please let me know what you think. I have very little money and no health insurance. So I need to have a real good idea of where to start now that im fed up with just 'living with it' and have to spend some money.

PS: I cant seem to pass gas naturally, I have to squat or lay on my side to pass gas by relaxing otherwise I have to bear down, like when passing a stool to get gas out.
PPS: Leaning to the right, sometimes relieves the feeling of pressure I get at the top of my rectal (column? im not sure how to refer to it)

Saw a doctor today. All of my blood work is within the reference range. I was impacted, and have a hernia. 125 $ and 10 minutes of time. The impaction has been cleared. I've quit soda altogether now (wicked headache) . Glad it wasn't something more serious

Hey cheatcode.
It's seemingly good news, however, the question is what not has been
ruled out by this basic investigation.
It costs a lot of $$$ to have thorough diagnostics done.
Your thyroid function could be low
and you need Free T3, Free T4 and Reverse T3 thyroid tests.
The standard thyroid test(s) TSH & T4 miss more times than not.
I'm glad you quit soda and should you be able to quit tobacco as well, you can set all the money you save aside for your health.
You can invest in better quality nutrition, like fresh vegetables & salads, pastured chicken, low mercury fish, organic legumes, etc.
Daily activity & exercise is very important and so is proper hydration throughout the day.

The picture you have painted of yourself is scary.
Why do you still smoke?
Why are you consuming so much sugar, when your weight is 350 Lbs.?
You know that too many of your calories are  from sugar
and from what calories are left, there's no way your bodily needs can be met, as far as nutrients/minerals and vitamins go.
One of these consequences might be reverse T3   (rt3) dominance,  due to possible deficiencies, which have to do with the low thyroid function.
Check iron, selenium, vit D, B12, B6, chromium, iodine, zinc etc and also low testosterone and HGH.

There's nobody who can help you enough, unless YOU decide to make some drastic lifestyle changes first!

I have cut back to one soda a day, reduced my smoking to half the frequency. That might not sound like much, but it's leaps for me. I'm working on it now. Last night I had a weird hunch and followed it. I took ibuprofen. I never take meds for stuff even otc , but I just felt so swollen I wanted to give it a shot
My lower back pain was gone within an hour, an hour after that I felt something large fall inside my left side
Like a lump of something. My guess was a big old bm stuck in there. I haven't passed anything of significance yet 6 hours later but I'm hopeful. So you suggest I get a blood workup.. is there a standard one I should pay for that covers these things, or do I need an array of tests? I hate to sound cheap about health I'm just so poor.
