Rectal problems

My stools are hard and difficult/painful to pass. On Clean-up, there is always a spot of blood on the paper. My anus is red raw and sore. I thought that it may be caused by medication, but then I have been on the same medication for a long time before this problem arose. I have tried Germoline, Sudocrem and Epaderm, (Not all at once) but they don't seem to be helping and may and could be exacerbating the problem. Any suggestions?
