I can't swallow my food, or burp, very painful and embarrassing.

The last couple of years I have been having some stomach issues but have increasingly gotten worse.  My early symptoms where:  heartburn, acid reflux, spitting up acid when I bent over, muscle cramps in the top of my shoulders when I ate (about 15 min) constant dysentery especially after eating, sometimes a quivering feeling around my stomach.  The foods that I have learned that give me the most trouble are:  Anything with sugar, anything with caffeine, cabbage, anything with a smoke flavor, steak, ham, turkey, bread, pickles, corn, cheddar, milk, yellow part of the egg, chocolate, juice, wine coolers are a few.  I have lived with these symptoms by taking prev-acid, over the  counter antacids, & meds for IBS.  But lately I have some new symptoms that are almost unbearable.  When I eat the food gets stuck in my throat, I can't swallow water, saliva, food, nothing...and I can't burp.  The pressure is so painful it feels like my chest is going to burst open.  My mouth waters profusely, and I try so hard to burp but I wont come up.  At this time I also get the severe muscle cramps in the top of my shoulders.  If I lean over and act like I am trying to throw up I can burp up a wad of food and saliva( no bile, it never makes it to my stomach)  and then I'm fine and I  can finish eating.  This is very embarrassing and I have to be careful when I am in public eating, I have had to been carried out of a restaurant like a little kid, people think I am having a seizure.  I have been tested for H pylori and it was negative, I have an appointment  to get a scope done.  It has been mentioned that I may have a hernia or gallbladder issues.  Can you help me with any useful information.  I normal can burp any other time with no problems and don't have a problem swallowing fluids, just when I am eating.  I am 32 5ft 5 and have went from 124 to 116lbs in just a few months.  We are trying to conceive also and I am worried that getting the right nutrition for me and the baby.
