Back pain mostly at night, can’t figure out cause

I am a 34 year old male otherwise in very good health, 5’9” and 170lb, exercise regularly.
For about the past year and a half I have had terrible back pain at night that awakens me a few hours into laying down to sleep. It first appeared out of the blue in February/march of 2022 with no known cause of what could have caused it. I have tried every position, body pillows, and new mattresses to no avail. It has has progressively gotten worse too since it began and now radiates to my abdominal region, almost feeling like my internal organs hurt.  I can also feel pain in my lower thoracic vertebrae when I touch the bone.
Recently in the past month I have been waking up with my arms and hands numb. Additionally in the past 6 months, probably due to a lack of sleep, I have been having very vivid dreams and sometimes can’t distinguish if certain events have happened in real life or my dreams, so I have to ask my wife if they really occurred.  I have also had strange episodes of almost fluttering sensations around my body mostly in my upper trunk and chest associated with an almost surreal feeling. Lastly I have been having this feeling that my left inner earlobe is wet or tingling.
I have seen my doctor multiple times about this, gone to physical therapy, and have gotten multiple sets of MRI first on my lumbar, then on my thoracic and lumbar with no suspected causes. I feel like my health in the past year has taken a downward spiral, but every one I see seem to say i am fine.
If anyone has experienced anything similar please your help is much appreciated!

Thank you all.

I had the tingling and numbness as one of my many symptoms of vitamin B12 deficiency. I would wake up with numbness and freak out shaking my arm for half and hour to get feeling back. I just thought I slept on my arm wrong. I would wake up thinking my ear would fall off due to affected nerves.

In the end my arm and hand was permanently numb 24/7. I started B12 just in time (close to death by this stage) and it took a year for 90% improvement. Some permanent damage. I have more permanent nerve damage that showed up earlier. Treatment window for B12 is 6 months after nerve symptoms showed up or permanent nerve damage. I personally did not have back pain from this deficiency but back pain is a possible symptom.

Check B12 serum, active B12, homocysteine and methymalonic acid. B12 serum alone can miss the diagnosis. Many have suffered with “normal” B12. The reference range is   too high in most countries.

Hello~Since you have had all kinds of tests and they have been "normal" I highly suggest seeing a chiropractor as your symptoms point to misplaced and pinched nerves and vertebrae. The chiro will look over the x-rays and determine exactly what is going on. He will discuss the plan of action. After a few treatments, you should start to feel better and all those symptoms will start to leave.

Thank you. I appreciate your help.
I, too, agree with her. I am currently under chiropractic care for migraine.
