Concern of STD after exposure with Thai prostitute

Hi Dr,

I am a 26-year old male.

I had a one-time sexual exposure with a Thai-prostitute and currently very concern about contracting HIV/STD.


On the 13th of Sep, I made a rather impulsive decision to phone for an out call escort. This is due to me never had any real sexual relation with other women and I wanted it so bad. I got the phone number online and I asked for a Thai escort to come to my residence for sexual services.

I brought her up to my room. She didn't take any shower and she quickly proceeded to taking off her clothes and undies. I saw some tattoo marks on the back of her body but no any obvious cuts/ injection marks.

As this was my first time having sex, I asked her for condom. She then helped to put one on my penis. She then asked me to relax on the bed and proceed to give oral sex on me (sucking my penis). I didn't remember I ejaculated.

A few times she also licked and bite both my nipples. No bleeding or injury from this act tho.

Afterward, she lied on the bed with her legs spread wide apart and I excitedly proceed to giving her unprotected cunnilingus. I only knew that unprotected oral sex can transmit HIV/STD after reading articles online. (And now I am worried) Her genitals area has no pubic hair (shaved) with no sores/cuts. I licked and tasted her vagina for a very long period of time. (about 5-10minutes). I think I may have also swallowed some vaginal fluid.

I then take off the previous condom from penis as I was feeling uncomfortable and asked her for a new condom. I remembered both condoms were rather cheap-looking red coloured one. Not sure if latex or not. She then put it on me and then we proceed to have protected vaginal intercourse. During the intercourse, she applied some lubricant to smooth the act. During intercourse, neither she nor I experienced any pain. I could not recall if the condom was broken during the act but after I reached orgasm, I pulled out and check.

I could still see the semen filling inside the condom. I presume there's no leakage or any hole in the condom. I then threw the condom away to the bin. After that, she quickly went to bathroom to wash herself and she did not ask me to wash myself!

This ended with her getting her driver back to her agency.

Her English is bad but I recalled I asked her if she has any STD and she said no, (I presume no any prostitute in the right mind would admit they have HIV/STD)

Also, she didn't asked me to take shower before having sex as well. This made me worry as if this is her normal practice with other clients, it would mean her sexual hygiene is not good..and might be high risk.


I have few questions below and hope that Dr you could enlighten me:

1. What are the risk that I will contract HIV /STD from this particular sexual intercourse? She is a Thai out call escort that is working in Malaysia at the moment. I paid a rather low money on this and the agency accepted..this made me think that she is likely a very cheap sexual worker and hence very high risk for STDs.

2. Does performing cunnilingus on a thai prostitute constitute a high risk in getting STD/ HIV? I also swallowed some of her vaginal fluid during the act.

3. I have went to my primary-care doctor on the following day for a blood test. When I asked her if she knew any medication to take she did not suggest PEP. Few days later I got the report. All shows negative including HIV and other STDs. My GP recommended to be tested again in 3 months time.

4. As it was my first sexual experience and a high-risk one, I did not know there were Post Exposure Prophylaxis (PEP). As it is not that easy to get from where I live. I only went to the local government hospital on the 3rd day..and the doctor told me that I have already passed the 72-hour window period for PEP to be effective...I felt really worried and regretful for not taking action earlier to seek PEP. If I were to take PEP, will this reduce my chances of contracting HIV?

5. Is HIV/STD common among Thai sex workers in Malaysia?

Please answer my question Doc as I am very worried about contracting STD/HIV from this exposure. I have made a rather stupid mistake by giving in to my impulse and should take more precaution next time.

I only realize most doctors latest comments were few years ago.

Oh well.

Is there any other forum or place I could ask my questions?

I would still need specific advice on my risk and what is the next plan of action

This hiv prevention forum is the only place on Medhelp that deals with HIV risk.
regarding advice, your doctor should be able to verify that you had no risk, and we are not affiliated with any other site.

Regarding HIV transmission in particular.

The doctors left years ago, but this is a moderated forum (wrong answers get deleted) and we rely on the advice of expert doctors.

No HIV risk. Only the head needs protection using the condom and if a condom failed you both would know it because it is a large rip down the seam and it is in tatters.
An HIV test or PEP would be purposeless.
HIV is instantly inactivated in air which means it is effectively dead so it can't infect from touching HIV or oral sex. It doesn't matter if you and they were bleeding at the time either because it is effectively dead.
Only adult risks are unprotected penetrating vaginal or anal sex or sharing needles that you inject with but you didn't do any of that.
Even with blood, lactation cuts, rashes, burns etc air does not allow inactivated virus to infect from touching HIV or oral. The above HIV science is 40 years old and very well established so nothing you can add will make it a risk.
