I forgot to mention that he might have rubbed his penis against the anal area before putting the condom on. I don't actually remember him doing it, but I'm completely sure that he did use a condom for penetration.

You were not at risk from your protected sex event. You would have known immediately if the condom broke- it is as overt as a balloon popping. You have nothing to worry about over these incidents and do not need testing.
Going forward, however, I suggest that in addition to using condoms, you also discuss your male partners' HIV status/testing history before getting involved with them, so you are not solely relying on the integrity of condoms for safety.
condom failure is catastrophic and you would have known without a doubt.
