Are women routinely tested during each pregnancy, or if they sign a refusal in the beginning, does that refusal apply to each subsequent pregnancy, as well?

The only way she did not test was to sign a refusal. If she didn't sign a refusal she was tested.

The doctors will not let her breast feed. Don't depend on her pregnancy to verify HIV status. I don't think it is mandatory to test while pregnant but most women do get tested to proctect their unborns.
