Labile Blood pressure with syncope

My 84 year old wife with Parkinson's and congestive heart failure has extremely labile blood pressure. She can have a systolic bp of 84 which leads to fainting and an hour later a systolic of 150!.  Her bp meds are Norvasc, 5 mg, 1/day, Coreg, 40 mg, 1/day; Enalapril, 20 mg, 2/day.

She has had 4 fainting episodes in the past 15 months and has been very lucky that she has not seriously hurt herself as she fell.  Her bp can vary from 120/70, to 160/90, to the 85 to 100 systolics which lead to fainting.

The combination of meds arose from doctors treating her in our winter home in Florida and our home in New Jersey. I believe that better bp control can be achieved using the wide range of bp meds available.  . I think some doctor has to start from scratch and develop a combination that works better than these.. We we can try a drug regimen for 3 weeks, taking bp at home and fine tune as we go alon, I believe.  Unfortunately all our cardiologists want to do is run echo cardiograms and stress tests.

Am I way off base in thinking that way? Opinions welcome.

Also, work on hydration and see if that helps, if the doctors think that's okay. Getting enough to drink can make a huge difference.

And avoid sudden or extreme changes in position.

See if you can get them to do a tilt-table test. Autonomic dysfunction aka dysautonomia is supposed to be more common in people with Parkinson's.
