Unsure what to do

I am 15 years old and for about half a year I have been suffering from the following symptoms:

Palpitations
Pounding blood in the ear
Slight shortness of breath
Cold hands and feet
Fits of sweating and fever cured with a glucose tablet
Fits of dizziness after a warm bath or shower (nearly blacking out)

They have gradually been getting worse. What could this be? Should I see a doctor?

Thanks.

Yep, you should see a doctor.  Probably a psychologist would be a good starting point.

I am also suffering from pain in my legs, diognosed as a muscle disorder but my right hip has suddenly become worse and has a different type of pain. I also occasionally suffer from pain in my chest.
