What causes drop and increase in HR with pacemaker.

My mother in law has had a pacemaker for 6 years.  Since receiving the PM, her average heart rate is between 80-90.  Suddenly her HR drops to 60 and stays there for a couple of weeks.  When this happens, she is completely wiped out.   The doctor doesn't seem to have any explanation for her.  HR  will stay at 60 for a couple of months and overnight it goes back to 80-90. She has been doing well  for a couple of months, and last night HR dropped again to 60.  What causes this to happen?

that does sound odd.  What is her setting at? (ie, how low does her HR need to go before the PM kicks in OR is she constantly paced? IF she's constantly paced it should certainly not fluctuate like that!)

Yes, she attaches an electrode to herself where her pacemaker is, and via telephone they check it from her home to the clinic.

I assume they have checked the pacemaker programming and that the leads are attached to the heart properly?
