Have the doctors done vitamin deficiency test? I had same problems and added Magnesium. 200-500mg/day and the pian and jitters were gone almost instantly.
There are a number of possible causes, and some can be as simple to fix as changing your diet.
You need to see a doctor for a basic evaluation, at least. Only an MD who has examined you can give you information about what is going on.

You should have yourself evaluated for the chest pain. It becomes all the more significant because of your associated symptoms of shortness of breath and palpitations. It could be due to arrhythmias (altered electrical system of the heart) or angina. When in doubt always best to have it checked. So please consult your primary care physician who may ask for an EKG or a treadmill test to rule out a cardiac cause for the symptoms.

I am sorry but to really get down to the problem you are going to have to have to get some investigations done. It will be hard for your doctor to decide course of treatment without knowing if there is any underlying issues.
Echocardiogram, holter, maybe stress test.
When you exercise are you feeling this, or worse?
Have you noticed foods that bother?
Fluid intake?
Start to take a journal of events and describe your eating, activity and situations when it occurs.
