Life is full of risks. Sure, hbvers have higher risk for cirrhosis and HCC, but there are medications that can successfully control hepatitis, slow or stop progression of fibrosis and hence greatly reduce the risk of HCC. Most people die with hepatitis, not from hepatitis. Who knows, a cure may be just around the corner or you may be hit by a bus around the corner.

Normally, HBeAg -ve indicates a non-replicative phase: patients have very low hbvdna, normal ALT , and are less infectious. You friend should have this confirmed by checking his viral load (hbvdna). He can stay in this phase for many years or he may develop HBeAg -ve chronic hepatitis which may then require treatment.

All hbvers can lead a normal life and marry. His partner should be vaccinated, and so should his children.

Your friend should have at least a yearly check of his liver functions (ALT,AST) and viral load; and get treatment when it is necessary. He should also avoid alcohol and lead a healthy life  just like everyone else.
