PS to James- Even in the setting of cirrhosis, I think the 3 month is pretty predictive, though cirrhotic do have higher relapse rates. I recall in my own (cirrhotic) situation, looking under every rock for relapse stats while waiting for my tests, (just to make myself totally neurotic) and I felt pretty good about it after clearing the 3 mos hurdle.

Nevertheless, I'd like to offer a big "Wah-Hoo!" even if it is a little premature. I did not know that relapse was more common with cirrhosis .. good to know, James & Bill. Thanks.
I should probably rephrase that. The 6 month test continues to formally determine SVR across all populations of HCV. I think testing at 30 and 90 days is less predictive of SVR, however, if cirrhosis is present.

I believe if cirrhosis is present, SVR is more accurately determined at 6 months post treatment; relapse is more frequent in that population. However, I think once 6 months has passed with continued undetectable viral load, the SVR status is considered durable.

The rest is most likely formality; recent studies suggest 90 days strongly correlates with 180 days, as long as cirrhosis or HIV coinfection aren’t part of the equation.
