Dosage is usually dependant on many factors . specifially weight , possible heart issues and the tolerability of the medication as well as how  one responds by doing blood tests

The normal ref range for TSH is .5-5 however many knowlegable Endocronologists are now looking at the optimam level to be somewhere .beteween .5 - 2 . I will search later for the articles .

At 65  your TSH is showing extreme Hypothyrooidism(which can cause many of the symtoms you are experincing) and I would hope you are being seen by a specialist (an Endocronologist)  in order to deal with  this correctly. as many general practioners and even Gastros do not seenm to have the knowledge on thyroid hormone problems.

best..

Hi I am so very very sorry to hear of your problems.  It is awful that your TSH was allowed to get that high.

Are you saying that you have been taking the thyroxine for 5 months and still have symptoms?
What does your doctor say?
If your TSH is this high, pretty much all of your symptoms could be from this.

It is possible that you have a disorder where the body can not convert the thyroxine to.  It would be an autoimmune problem which I think is Haishimoto thyroiditis but I can't remember so I will go look
Hang in there

I take levothyroxin and cytomel.  I will try to find some help and come back,
