Low Hemoglobin and High Creatine

My Dad who is 81, has been healthy most of his life but for last few week having breathing problem. Three weeks back we took him to doctor and doctor found that he is deficient in Iron, Weak Kidney and small blockage in heart.Doctor suggested to for iron dip and gave some medication for kidney. His current hemoglobin is 7.6 and Creatine has gone up to 3.6 . And now he is not keeping well. Any advice what treatment can be done?
