Hello, at first I thought this was a typo, I am searching for some kind of information. I did find this so far, I will be back.
1a - mostly found in North & South America; also common in Australia
1b - mostly found in Europe and Asia.
2a - is the most common genotype 2 in Japan and China.
2b - is the most common genotype 2 in the U.S. and Northern Europe.
2c - the most common genotype 2 in Western and Southern Europe.
3a - highly prevalent here in Australia (40% of cases) and South Asia.
4a - highly prevalent in Egypt
4c - highly prevalent in Central Africa
5a - highly prevalent only in South Africa
6a - restricted to Hong Kong, Macau and Vietnam
7a and 7b - common in Thailand
8a, 8b & 9a - prevalent in Vietnam
10a & 11a - found in Indonesia
Hi I found an article you can read about it. There is a new drug that may be approved in December or shortly thereafter.
I am sending you the URL so you can take a look. I am having trouble finding the K but there is a lot of information regarding Genotype 3. I hope this helps

How do you get That sofosbuvir? Coz im scared of the Side effect of interferon which sounded like opiate withdrawal and yes i have depression im an x addict and i dont want to be hurting coz its like a nightmare to be hurting again like That for 24weeks. Does the interferon causes the Side effect or the ribavirin?

Welcome to the forum,
I have not heard of genotype 3k, maybe a typo. I have genotype 3 as well and am on treatment of interferon and Ribavirin.
Sofosbuvir is not yet approved for use by FDA but in process and expected to be as early as December this year. Very soon!

List of genotypes:
Genotype 1a
Genotype 1b
Genotype 2a, 2b, 2c & 2d
Genotype 3a, 3b, 3c, 3d, 3e & 3f
Genotype 4a, 4b, 4c, 4d, 4e, 4f, 4g, 4h, 4i & 4j
Genotype 5a
Genotype 6a
Okay I have found reference to your genotype:
The biochip contains 120 oligonucleotides that identify genotypes 1 to 6 and 36 (1a, 1b, 1c, 1d, 1e, 2a, 2b, 2c, 2d, 2i, 2j, 2k, 2l, 2m, 3a, 3b, 3k, 4a, 4c, 4d, 4f, 4h, 4i, 4k, 4n, 4o, 4p, 4r, 4t, 5a, 6a, 6b, 6d, 6g, 6h, and 6k) subtypes.
And as well 7,8 & 9, that Dee referenced.
So is not a typo! But you could find lots of refernce to genotype 3 and be sure that treatment protocol would be the same.
Apparently 3k is common among blood donors in Indonesia.
Im not sure but I will go through my bookmarks after this to verify but I believe that genotype 3k used to be genotype 10a but was reclassified. 3k is most prevalent in Indonesia and Iran if my memory serves right. Again, I will look after I post this.
Prior to 1994, new variants were identified in Vietnam, Thailand, Burma and Indonesia. These viral genotypes were originally classified as genotypes 7, 8, 9, 10 and 11. These variants were reclassified in 2005; genotype 10a came to be denominated subtype 3k, and genotypes 7a, 7d, 7b, 7e/7c, 11a, 9a, 9b, 9c, 8b and 8a became subtypes 6e, 6c, 6d, 6f, 6g, 6h, 6i, 6j, 6k and 6l, respectively [1].

Besides being a Genotype 3k, how is your liver? Have you had a biopsy? If so do you have any fibrosis? Has your doctor mentioned fatty liver (hepatic steatosis)? What is your viral load? I see you are <40 so that is a plus. A little more information can be helpful.
Sofosbuvir will hopefully be on the market soon but the only plus side for a Genotype 3 with that is the fact you will not have to take interferon. Studies have shown it to be not as effective for a G3 as it is on others.
EASL 2013: Sofosbuvir + Ribavirin Cures Most Genotype 2 Hepatitis C, but Genotype 3 Response Is Lower

You are correct in anticipating the possibility of depression type feelings with interferon therapy. A lot of doctors even start their patients on an SSRI type anti-depressant like Prozac a couple weeks before starting treatment. It's not quite as bad as it sounds, though some patients do describe it (at least a couple days each week) as being similar to withdrawal. The SSRIs actually seem to make this more bearable on a physical basis and are not just a happy pill.
If you do choose to go with the current standard of care (pegylated IFN and ribavirin, and yes it's the IFN that causes the depression and 'flulike' feelings) there are some additional tests like the IL28B that can help guide your treatment decisions as you go along.
Good luck whatever you decide.

(and yeah, I used to get the 'typo' thing too)
I agree with what others have said. However, I would add that you should really get a doctor who is knowledgeable in the treatment of all types of Hepatitis C (all different Genotypes). If I was you, I would get a Hepatologist (liver specialist). A Hepatologist will be knowledgeable about the currently available treatment and also about the new treatments coming in late 2013 - early 2014 (if they are approved). Because you are young and because of your several concerns, which you mentioned in your previous thread, you may decide to opt to wait for the newer treatments which do not contain Interferon. A Hepatologist can discuss your liver fibrosis stage with you and can discuss your treatment concerns and then help you make the best treatment decision.
The doctor havent ordered me some liver biopsy yet, im scared of That putting a needle to get a sample does it hurt?. My viral count is dropping to 3 million something from 5 million when first diagnosed. but they did ultrasound the other day and everything is normal thank GOD. So if i take sofosbuvir and RBVirin will it causes like a withdrawal symptoms. My addiction md wants to continue putting me on suboxone he say if i Go with the interferon treatment cause he dont want me to feel the pain as withdrawal does, for x addicts out there u guys know how it feels even thinking about it makes me scared
And you guys are right im indonesian
So i got it from there weird cause ive been here for almost 8 years and no symptoms at all
How is the treatment feels like to you? can you work while on it? Is the pain really bad? Can u sleep?
Nio,
In regards to treatment SX, i hear that some people breeze through treatment, without any sides, however most people have some side effects. . .
I am also type 3 and on current SOC, Interferon and Ribavirin. So far I have good and not so good days. In the first few weeks i had mild flu like symptoms. Then through weeks 3 - 8 I had low blood hemaglobin issues, that were relieved with a rescue drug (Eprex , in Canada). I am now feeling much better and can work. However during low blood issues, i was really effected by the exhaustion and could not do much more than sit around.
My thinking is effected somewhat, I get frustrated easily and loose my cool.
If I was to compair, some of the physical symtoms are only similar to heroin withdrawl, but more like just having the flu, and without the major anxiety of needing to use to relieve symtoms. The main difference, for me, being the thought process of putting up with some pain or symptoms to kill the virus. As tylenol and antihistamines can relieve a lot of symtoms - it is different. Addiction is not just a physical reaction, it is phsycological. The mental thought process is different and therefore the symtoms are different than detox.
As you might expect, there are people on this forum that have detoxed and gone through treatment. Personnally I'd rather do treatment for 24 weeks than detox for 2 weeks.
Good luck to you, you have come to the right place!

"My addiction md wants to continue putting me on suboxone he say if i Go with the interferon treatment cause he dont want me to feel the pain as withdrawal does, for x addicts out there u guys know how it feels even thinking about it makes me scared "
Im an ex-addict who went through Interferon treatment. I finished it in April of this year. Im really not understanding what your addiction MD is telling you. Interferon is NOTHING like detoxing from drugs. Id compare it more to having flu symptoms...drug detox is a lot more hardcore. And with the Sofosbuvir, Ive read that its much easier than interferon. Unless your addiction doc did treatment himself, dont let him fill your head with things that scare you.
It seems like you have a lot of anxiety about the whole situation and you are looking for the easiest and painstakingly free way to go about this. That makes sense...I mean who wants to feel bad if they can avoid it right? Look, theres no guarantee you will have a hard time on treatment...in fact, many get through it alright. You are young so you have that going for you and you only have to treat for 24 weeks versus if you were a G1 you might have to go 48 weeks.
You may be able to wait for Sofosbuvir to get approved....but nothing in this life is guaranteed. You have genotype 3 which is the fastest progressing of all the genotypes.
Journal of Viral Hepatitis Role of Hepatitis C Virus Genotype 3 in Liver Fibrosis Progression
"The progression of liver fibrosis in chronic hepatitis C has long been considered to be independent from viral genotypes. However, recent studies suggest an association between Hepatitis C virus (HCV) genotype 3 and accelerated liver disease progression."

You don't know when you contracted HCV so you dont know how long youve had it. You havent had a biopsy so you truly have no idea how your liver looks. Im not trying to add to your anxiety or scare you in any way but rather educate you. If your worried about if a biopsy hurts or the side effects of treatment etc, maybe look a little more longterm. I posted above how Sofosbuvir hasnt shown that great results for a G3. These are things to consider. You mentioned your viral load going down from 5 to 3 million....it will continue to fluctuate like that. One minute my viral load was 2 million and then two moths later it was 12million. Anything >400,000 is considered high. And keep in mind, its about your liver....not your viral load.
Are you currently on Suboxone or is your dr talking about putting you on it? I really dont understand what suboxone has to do with HCV treatment. Could you be more specific. You need to find a Gastroenterologist or a Hepatologist to treat and advise you on your HCV..not an addiction specialist who it sounds like is feeding your anxiety with BS.
I have to agree with previous comment from ABN, you need to find a specialist, with experience treating hepatitis C.
In Canada you cannot be treated until you have been drug and alcohol free, if dealing with addiction issues, for at least 6 months. This would give a person some time to get over some of the anxiety and risk of relapse.
Really be sure you are ready to begin treatment, find a good hep doc, and commit to it.
Thanks guys and gals for the replies, yeah i am full of anxiety, scared. So if geno 3k is from indonesia meaning ive been having it for a long time cause ive been here for like 8 years and never had any symptoms, and according to ultrasound everything is normal. Is there any New tech for liver biopsy thats uninvasive?. And so i contacted my Insurance Kaiser to request for a hepatologist not gastroenterologist since That doc is only an NP not That i am underestimating NPs but its making me more nervous you know. In the mean time my plan is take milk thistle twice a day, curcumin twice a day and see how my next bloodwork (next month) goes hopefully it will kill the virus. and in the mean time find a hepatoligist. Any of you use herbs?
"Is there any New tech for liver biopsy thats uninvasive?"
Yes
You can ask for a FibroScan or a FibroSure which is a blood test.
"In the mean time my plan is take milk thistle twice a day, curcumin twice a day and see how my next bloodwork (next month) goes hopefully it will kill the virus."
If milk thistle and curcumin killed Hep C then there would be no use for interferon or new trial drugs.
The liver biopsy is not that bad and it is the most accurate test to determine your liver fibrosis stage. You really do want to know what stage your liver is at so you can make the most informed decisions about your treatment. They can give you am IV sedative, which does not knock you out, but you won't feel any pain from the biopsy if you have the sedative. The biopsy takes only minutes to do.
Milk thistle and curcumin will not cure your Hepatitis C. Only the approved treatments work to cure Hepatitis C. Acting Brand New hit the nail on the head in saying that, "If milk thistle and curcumin killed Hep C then there would be no use for interferon or new trial drugs." I would venture to say that not one of us would have done the current treatments if we could have been cured with milk thistle and curcumin.
PS: My ultrasound was perfectly normal too. My liver biopsy showed Grade 2 (inflammation) and Stage 2 (fibrosis).
