2 log drop

I dont remember how the math works. I may start treatment next week. my VL is 201721H <43 IU/mL and or 5.30H <1.63 log IU/mL. Ok so they look for a 2 log drop. Is that a week, mo or 8 or 12 weeks.  What would my VL be with a 2 log drop. confused. Thanks

The standards are changing, in part because new drugs are going to be available in the next year or so. I think unless a person is at a stage of the disease where they can't wait or they just won't accept the facts and wait for better drug, most good hepatologists would suggest stopping at week 12 if not und or very very close to und.

Most of the new trials will stop at week 12 if you are not und. At this point the standard has been set by some of the new pi's and to compete a new drug is going to have to provide early und and svr after 24-28 weeks of treatment.

If you have geno 3, since the most of the new drugs don't seem to any more effective then soc, that might be another reason to continue past 12 weeks. I don't know a lot about treating geno 3 personally.

Thank you again. I would wait if I thought I would have health ins. and I thought something would come along better. As it is now this study has only accepted 39 people in the U..S or the world cant remember. I have a almost 67% chance of getting this PI made by BMS. They will give 48 wks tx if you only get SOC and your logs drop the way there suspose to. Im thinking if I dont become undetected By 12 and I need to I can break out my BlueCross and Make a mad dash to continue Tx and get as far as I can before I have to worry about ins. Maybe I be making enough Doe by then for Cobra. The other day I got express employment from the Dollar Store. Their insurance said if I had a previous condition they would not cover it. It also said if I had a condition for 1 yr prior to diagnoises they would not cover it. I did'nt get the job anyways butt.....Untill laws change everyone in the working world with this economy could find theirself in a simular position..Maybe Obama will change it (pre-existing conditions) maybe he wont. Maybe a better trial will come and maybe it wont. I just hope this new drug is better than Tela, Boca? and BMS790052?  And not a cheap know off that does not work worth a crud.  I can only hope that all drugs get better. We know Tela is wonderfull and We know That Boeciept(you know what I mean that B miricle pill) is too.. It does concern me that nygirl had a low viral load and had to tx sooo long. and as mentioned by some one so did bill195young did too. I can only hope to hear of someone with a low VL that cleared huh? I do wonder if they have been trying out tela and the likes for a long time and if they have passed the test of time?   Thanks everyone...With kindest regards I am Ginger l Cormier

yes Deb (nygirl7) used the riba sledge hammer approach along with a grueling very long tx. When you have two geno's, later stage disease and no new drugs on the horizon, sometimes that is the best course of action.

As I was just saying to spectda I am geno1 and to be honest don"t know weather AorB or both but I will ask that tommorow when I go for labs.I am feeling positive at this early point in tx.  and as  one of the new members of this site I will look to guidance from you and others  that have been around here for some time! And thats great that yours is GONE!!

Which geno are you Will? Sorry I should know but I never can remember these things and I dont see it in your profile.

Whichever you are just remember you can do it - I was both a geno 1A and geno 1B and they are both GONE.

You have to at least have a two log drop at week 12 to continue onwards but then preferably you want to be UND at week 12 so you dont have to extend.

If you have a 2 log drop but are not UND at week 12 you can continue to week 24 to see if you can clear but that is the drop dead date (sorry about that expression) and at that point if you are not clear you need to stop because the odds of clearing - even if you extend - are like 1% or something absurdly low.

And if you are not clear at week 12 but you are clear at week 24 if you are a geno 1 you need to extend to week 72 for the best chance you can get. (From the Sanchez Tapias study).

But of course the Berg study showed us that being UND at week 4 gives the best best best odds of success like 90%) so earlier is better of course but any way you get to SVR is great!

I am a little confused to,is it to be UND at wk.12 that you look for or a 2 log drop at wk12 in order  to consider cont. with tx.appreciate if you could explain that ???

PS at 200,000 you have a low starting viral load. Mine was 568,000 and it was considered low (yet I had a harder time clearing and did not clear till after 12 but before 24 so I had to do 72 weeks, ugh). Still it worked.

But again........you'll get the good stuff and just wipe it all out fast!  :D

Blondie to do the math just drop a zero for each log
20,172 one log drop
2,017 two log drop

Usually you look for a two log drop by week 12 to be able to stay on treatment if you are still UND - but you should be UND by then so you don't have to extend.

You will get the trial drug though (I am hopeful) so it should be no sweat. Im not sure what their specific criteria is what I gave you was for SOC.
