Herpes 1

Good afternoon,

I will make this brief for the experts on here since I know most of the questions are repetitive for you and I don't want this to be one of them. I had a high risk exposure, went on PEP, tested but have a lingering symptom that is worrying to me. I have a "sore" throat for over a month now. Itt does not hurt but is red along the back wall of my mouth and uvula. There is also small grey spot in one area and it's now starting to burn a little at random times. No pain while swallowing.

My exposure was insertive anal on a trans. Condom must have broke mid way through and I did not realize. I finished and did not notice until I pulled out. I have known this person for 10 years and it has always been protected. They state they are clean but would not prove anything to me or even test when I tried talking which is disturbing. They don't have the best past and have had MULTIPLE partners.

See my timeline below:

7/19 - Exposure
7/21 - Started PEP
8/17 - Finished PEP
8/18 - 4th gen test negative (30 days post exp and 1 day post pep)
9/5 - 4th gen test negative (48 days post exp and 19 post pep)
9/5 - Full STD panel - all negative except for Herpes 1

I am testing on 9/18 (60 days post exp & 32 post pep) and again on 10/2 (75 day post exp & 46 post pep).

The only other thing I want to mention is I did receive a "rash" on both my hands for roughly two weeks in the middle of pep or end of pep. Small red dots that looked like they were under my skin, not raised and did not hurt but where just there. My hands were also extremely dry with occasional small white dots of dry skin. As I said it lasted for roughly 2 weeks and disappeared.

Besides that, I have experienced nothing else physical. At this point a lot of mental stress. Can my "symptoms" be from a recent Herpes 1 infection ? The information I find does not point to that which makes me think it might be something else. I have a follow up with my doctor but was wondering if these are linked. Unfortunately, my doctor is not the best but that's a separate issue.

Do you have an index number on your hsv1 test? It should say something like 4.92, or 11.8, or 33.45, etc.

It is quite possible that you had oral hsv1 prior to this event. 67% of people under 50 globally do, and the vast majority never get any symptoms so they never know they have it.

Herpes wouldn't give you symptoms on your hands and feet. Did you get a syphilis test? That would be listed as an RPR, VDRL, FTA, TP-PA, or EIA. It might have the words non-treponemal or treponemal in it. Your rash doesn't sound a lot like a syphilis rash, but it's good to know if that test was negative.

I know you are scared right now, but multiple partners doesn't mean huge risk they have always used protection. Try to remain calm here. You judged your friend as worthy to have sex with before the condom broke, and that doesn't change because it did.

I say that not to judge you or lecture, but to hopefully help calm you.

Just to clarify, we have always used protection at my request,  but I'm not too confident with her multiple partners. She asked me to skip it more then once.

I had a full STD panel and it was all negative except Herpes 1 and the number is 3.91
Is this a sex worker situation? Most sex workers use protection to protect themselves from you, not the other way around. If it's not a sex worker situation, and more of a friend thing, maybe she just trusts you.

Your 3.91 is likely an established infection, meaning that you had it prior to the encounter, though that's not hard science. In any case, it's not likely to cause your symptoms.

Has a doctor seen your throat and rash? It could be from the PEP.
So 3.91 means prior ? I was confused as I assumed it was new and the higher number was established. Thank you for that.

My rash disappeared within two weeks and no one saw it. My throat was blown off as "viral". by my doctor since common swabs were negative. The fact that my two 4th gen tests came back negative, my doctor is blowing off the thought of HIV as respectfully as he can.

Don't mean to sound negative but unfortunately, this is all just too much of a coincidence as this came 2-3 weeks after my exposure. This was my only "unprotected" sexual exposure.
It's not an exact science. If it was a bit lower, I'd think maybe it was a new infection, and it may be - there's no hard science behind this. Even still, an hsv1 infection is rarely transmitted from genitals to genitals, and wouldn't cause a rash on your body.

It would be transmitted by kissing or oral sex.

There are so many viruses going around right now - covid, RSV, the flu, adenovirus, etc., that all of this could be just "viral". Remember that you were mostly protected, and you don't really know how long it was unprotected.
