Dr. please help!

Dr.,

I am a 25 y/o male, and have only been sexually active with only 5 women. I have always been very safe and always used condoms. I had recently broken up with my previous girlfriend of 6 years and I guess you could say I was naive. Unfortunately, with my new girlfriend I wasn't as careful. We had briefly talked about STD's and she reassured me that she had recently been check and was given a clean bill of health. After 2 months of dating, we had unprotected sex. Everything was fine. Over the next couple weeks we had unprotected sex two more times. Two days after the most recent experience, I notice that I had a 3 red spots on the head of my penis. To give you a better idea, it looked like one of the scales of skin was bright red. Initially, i was concerned but it went away a day or two later.

To be safe, I made a appointment for my pcp; unfortunately, it would be a month before i would be seen. So after waiting a couple weeks and decided to go to a local STD clinic (5/30/2008). I tested nonreactive for syphilis, negative for herpeselect-2 elisa igg(HSV-2), and negative for chlamydia.They would not test me for HIV because i was not a high risk. She also made an appointment at a different clinic. She had a normal pap, tested negative for syphilis, and HSV-2. I am not sure why she wasn't tested for anything else, like HIV, but I assume it was due to cost.

A month later, my girlfriend, who had just gotten over viral laryngitis, performed oral sex on me. Over the next couple days, i developed my first urinary infection--worst experience! I saw a NP and he told me that is was most likely due to the viral laryngitis. He gave me a 3 day (2x day) course of Cipro, why? It did not help, but after a week of pain the UTI finally went away. However, just a precaution my pcp referred me to see the urologist (scheduled on 8/18/2008)

As a result of this whole ordeal, I have become very vigilant of my genital area. I started examining my penis with a very bright lamp. I notice that the skin on the head of my penis had become bumper than I remember. To give you an idea, I would say that it appear to look like sandpaper. However, it is bumpiest near the urethral opening. I notice this texture during mid-July. On August 4, i went back to the STD clinic and showed the NP the texture change. She told me "sometimes your skin changes." She also stated that it was nothing to worry about and that i shouldn't   "go looking for things that aren't there." I completely agree. I had become very stressed due to work, studying for the MCAT, and a paper I began writing with my PI.

Just this weekend, my penis had become irritated and begun to ich, this was not in my head. The sandpaper texture hasn't changed, but the area of skin underneath and on edge of the crown of my penis head, appeared to be getting more like sandpaper. The skin on the crown appeared to be bumpier and the bumps (like sandpaper) went almost all the way around, where the skin underneath was smooth or at least i thought. Also, one spot on the edge of the crown had turned whitish color. Scared, I had begun to wash my genitals with a separate bar of soap (dial). Other than that I haven't changed soaps, underwear, or done anything out of the ordinary. I could not understand why my penis had become very sensitive all of a sudden.

Fortunately, I had my urologist appointment this Monday. She look over my penis and said that it looked a little irritated and that the bumps around the side and underneth were most likely from irritation. She did tell me that if any of the bumps grew, I should come back and see her. She told me to put some neosporin on it before i went to bed, to use Dove soap, and to switch to brief instead of boxers (which i have worn since i was 12). Just last night, the irritation had become worse. I carefully and genitally examined my penis. Upon pulling the skin tight on one area of the crown, I notice that everything was flat except for this oval, flesh colored, raised 1/32 of an inch bump. The NP did not notice this. I wish i would have noticed this and showed her. It has only been a couple of days seen I saw the NP, but the irritation has become worse and I don't know what else to do?

Some additional questions:

1. Could the original red mark and the resulting texture change be due an initial HPV infection? A ridiculous question, i know.

2. Do you think that the skin texture changes (both head and crown) sound like possible early warts from HPV? How fast do warts grow? Within a week or months? I have read that they are different in everyone. In, some people they grow large and in others the stay small.

3. Will a normal pap exam confirm that my girlfriend is HPV free?

4. Would you recommend i do anything to improve the irritation? If symptoms do not improve, how long should i wait before seeing the Dr. again?

5. Do you think I am overreacting? Maybe due to stress from work and MCAT?

6. Do you think my symptoms warrant a visit to the Dermatologist?

Doctors do not post on this forum. I want to give you my best opinion based on your history. This does not sound like HPV from your description. You SO having a normal PAP test is very reassuring. I think you are overly examining the normal skin found on a penis. And yes you are stressing way to much over this. Good luck with the MCAT and your application to med school. There you will learn more about compulsive behaivor in your Psych lectures. Relax you are fine and I feel your girlfriends is also. Take care.
