Groin Pain no swelling/lumps.

I have had pain in the groin for 4 years . It is on a lymph node near the femoral artery . It is sensitive to touch . I have pain when walking that can get intense and go into my genitals and thigh . I have to stop walking to ease the pain . Some days are better than others . It is also intensified by drinking alcohol . I have had the nerves , muscles , bones , veins , arteries , and gynocological causes checked . The only thing they haven't looked for is the nodes which is where the pain is coming from . But there is no swelling , only pain . There was some swelling before but not now . I also lost a lot of weight unintentionally when all this first started . I am wondering if it could be a low grade lymphoma . Is it possible to have lymphoma without swelling of the nodes ?I am due to have an ultrasound scan in a few weeks .

Hi, I don't think that you fit with an auto-inflammatory condition -  I was really using that as an example of how the immune system can do strange things that lie outside of a normal infection scenario.

Then again, the sporadic nature of your pain might be related to some general immune signalling - unless there is sporadic minor trauma..

Yes, the timing of your alcohol effects argue against vasodilation.

Thanks for your replies . I will ask the doctor about inflammation and auto inflammatory conditions . I wonder if such conditions could cause this pain for so long?.

I did test out the effect of alcohol again last night (I don't drink usually because of my liver disease) ,I had 1 glass of wine and it did cause me to have an ache some time afterwards (2 hours) and I still could feel it the next morning ,so I don't think its due to vasodilation.

My lymphocytes are allways down not up . And I don't think I have thick blood particularly although I am taking some medicine and Chinese herbs for my heart (I have heart disease aswell!) which thin the blood . But anyway I thought that if I had the WM it would show up on my blood tests .But I read that people with Hep C are not only more at risk of developing WM but low grade Hodgkins lymphoma aswell .

Yeah I think whatever is causing the problem on the lymph node , is pressing against a nerve ,due to its location right on the femoral nerve . But its not allways painful sometimes I can walk a reasonable distance without much pain other times its intolerable .

Anyway I hope I get some answers soon , and I hope the ultrasound is an accurate method of diagnosis.

"Plausible mechanisms for a role of chronic HCV infection in the development of WM include both cytokine-mediated and direct viral stimulation of B-lymphocytes, triggering clonal proliferation and progression to lymphoid malignancy in some patients. However, there is disagreement in the literature as to whether this hypothetical association between HCV and WM exists in practice"

So maybe your HCV is stimulating your B-Cells to proliferate, which may be  malignant or not.

And btw, your spot of swelling (which is probably but not certainly a node) is probably pressing on nerve, and it hurts more when the swelling gets a little bigger. There was a man here who had something similar, but unfortunately I can't remember his name. He'd been to several doctors also. He was non-malignant in the end.

Only a biopsy can say for sure - and rarely that's not even sure.

Maybe you have some granulatomous condition with inflammation that waxes and wanes and also gets acutely aggravated by factors like alcohol or pressing.


Have you noticed if your blood is particularly thick or thin?

Yes, absolutely. Cancer can do almost anything, and the same goes for the immune system.

that would seem to tend against cancer, and towards inflammation. There are auto-inflammatory conditions that do not involve infection.

It is known that in Hodgkins Lymphoma, nodes can ache from alcohol consumption. But there is a long thread here with people who get the same effect, but don't have HL. One possible mechanism either way involves vasodilation. When swollen nodes ache, that's usually because the internal swelling presses against the enclosing capsule and causes pain. Bradykinin is probably involved.

So possibly alcohol induced vasodilation is also involved in your case, pushing chronic swelling just over the threshold into pain.

There can be lymphoma with surrounding inflammation, too.

All of these things refer to tendencies and probabilities. The presence of a fatty hilum seen in a scan *almost* always means there is no lymphoma  and vice versa.

p.s why should I ask the sono tech if they see "fatty helium" .?

Hi Ken

Yes I understand they are not usually painful (nodes) but seeing as there are so many different types is it possible that some people do feel pain ?

I don't think its an infection , because its been so long and also because sometimes I can have pain on the left side also .

I don't understand how vasodilation from alcohol would make my groin ache .

I am worried about lymphoma because the pain and sensitivity is right on the nodes and they have ruled out everything else .

I also have hepatits C (for the past 16 years) which can make it 30 times more likely to get low grade lymphoma and 300 times more likely to get Waldenstroms  lymphoma apparently .

But yes I sure hope its something else entirely and I hope I find out soon .

Hi, lymphoma nodes usually aren't painful.

It's possible for a node to be infected, which can often be painful but that's from swelling.

4 years would mean a very, very slow growing lymphoma - and lymphoma wouldn't go down all by itself anyway.

maybe from vasodilation, and so histamine from inflammation might also be a factor for you

yes, e.g. there is MALT in the intestine.

But I would be thinking of other causes instead of worrying about lymphoma. Also ask the sono tech if they see a "fatty hilum". Good luck.
