If the pain in the testes persists, you may have to go back to the urologist to do an ultrasound scan to make sure everything is fine.
Take pain relief medication as prescribed on the packet.
For the penis head irritation, you do need to follow my advice I gave you in my first post.
Don't have any sex at all until and don't masturbate until you are well healed.
If you haven't been to see your doctor to get a swab to be taken from the genital area to be sent to the laboratory, then this is what you need to do.
These genital skin irritations can take a while to settle down, so don't stop using your antifungal medication. Be careful with cortisone creams because these can thin the skin.
You may need more than 1 dose of the Fluconazole and your doctor can prescribe a longer course for you. You do need to be careful with these as they can damage your liver and kidney, so only use a long term course by direction of your doctor.
The other thing that you can do that I did not mention before is to only use non biological washing detergents.
You have not been applying the antifungal cream on for long enough.
I have already been to a urologist twice,,first time he gave me antibiotics, second time I returned to him and told him I have an irritated penis head, and my testes hurt...He basically told me nothing...He said I don't have an infection anymore...

If you have painful testes, you need to get back to see your doctor urgently, with a referral to the urologist if antibiotics do not clear it up.

You can purchase 100mg Fluconazole that is a 1 single dose treatment without a prescription from the pharmacy. You will need to check with the pharmacist as I do not know which country you are in. You can also purchase antifungal creams specifically to apply to the genitals.
Your doctor can prescribe a longer course of the antifungals if you need them.
It is very common to get fungal infections when using antibiotics.
Don't have any sex until you are well clear. You can purchase latex free condoms which may be better for instead of the latex ones.

thanks for the advice, I was just doing some research and I have been on antiboitics for 6 weeks, zithromax, doxy, cipro, and maybe I have some kind of yeast infection or fungal infection on my penis as you mentioned.

Make an appointment to see your doctor to get a swab taken from the penis to check for any bacteria or fungal problems.
As you have already been on an antibiotic, it is likely that you have a fungal problem.
You can buy antifungal creams from the pharmacy without a prescription specifically for use on the genitals, but I would advice a swab test.
Don't use soaps, shower or bath gels to wash your genitals. Use soaps that do not have any perfume, or ones that have a pH level like our skin. Baby washes should be fine.
Wear cotton underwear and nothing that is too tight.
As a home remedy try applying some natural set yogurt to your washed and gently dried penis.
You may also benefit from using an emollient that you can purchase from the pharmacy without a prescription that you can use as a substitute for soap and it will also moisturise the area.
