I need Help

deborah0904 posted a site where you could submit a complaint in regards to HIPAA violations.  I filled out the PDF form.  You can't save a copy so I don't know how to submit it.  It keeps saying you can email it but I can't see how.  If course I am fatigued and have brain fog.  Can someone help? :0)

That would work.  I will have to hook the scanner back up or I guess I could send it snail mail.  Thanks for helping.

An idea:

You can fill out the form, print it and then scan it back into your computer. Send it as an attachment in an e-mail.

Sunny I will get an email to them to see if they can tell ow to mail the form.  You would think it would have been included in the packet.  Since you can not save the form I just can't figure out the how of it.  Thanks for being so kind.  I appreciate it.

Next, here is the e-mail for the general help... you can e-mail them and see if they will give you an e-mail address too. Its about halfway down the page and it's in the sentence that says  "ANYONE CAN FILE! - Anyone can file written complaints with OCR. We recommend that you use the OCR Health Information Privacy Complaint Form Package. You can request a copy of this form from an OCR regional office. If you need help filing a complaint or have a question about the complaint or consent forms, please email OCR at ***@****."
