When I had my LP, I had MRIs showing only one , solitary, out-of-place brain lesion, an atypical history with only one "attack" but a lot of symptoms that appeared and never went away, and a physical exam that was VERY suggestive of MS.  So the neurologist needed more evidence.

My results:

Cell count - lost
CSF glucose - normal
CSF total protein - mild to moderately increased - 71
IgG index - elevated
O-Bands - 1

My neurologist would have liked to see more O-Bands, but felt that this, combined with my other findings, gave him a greater than 90% confidence that I had MS.  Subsequent MRIs on higher power machines have shown many more lesions.

Does that help?

My LP that was just done in Dec. was normal; but I had lesions that showed up on my spinal MRI that sort of "clinched" the deal of MS.  I agree with what Heather says; it is used to point toward something but cannot be used to rule out anything if it turns up normal.  Like she says, the LP can be normal at maybe an early stage but eventually may show "O" bands.  I, personally, will NEVER go through the LP process again.  I ain't that curious!  God bless you, Amy

My spinal tap result showed the typical oligloconal banding (sp?) that is normally seen in people with disorders that cause inflammation in the central nervous system.  Not all people with definite MS will have these bands, but my understanding is that they will eventually show up, at some point in the disease process.  If your symptoms, relapses and remissions and physical findings on exam point toward MS, then I would be a little hesitant about a Neuro that says they won't diagnose MS without an LP.

LP's used to be used quite frequently, but when MRI came along, doctor's used LP's less and less to make a definitive diagnosis.  What did your LP's show and how did your experience go, having the procedure?  We learn alot on the forum from what other's experience.  So hearing your experience, may be helpful for others.  Thanks for posting.

Feel well,
Heather

I might be confused but I thought you had already been diagnosed with PPMS.  Are your doctor's now saying that they are taking "away" the diagnosis until you have a spinal tap?

Here is a specific link about LP's and what they are looking for to obtain more information that could lead to a diagnosis of MS.  Hope this is what you meant by your question.

Lumbar punctures are helpful in showing further evidence of inflammation going on in the central nervous system, but they are not critical to diagnosing MS; as a percentage of people with definite MS, have a clear lumbar puncture.
