Dizzy

For the last 10-12 days i have been experiencing dizziness and nausea, starts with a tingling warm sensation in my upper back and moves up the back of my head, at times down both of my arms, i get extremely dizzy (to the point i stagger) and for lack of better explanation i get dumb...cant concentrate, sow and sluggish.  I should back up and tell you that this have been happening for approx the last two years.  Had MRI/MRA of head and neck and the results showed nothing.  Have been the emergency room several times with the same complaint.  It seems like when this happens my blood pressure gets high.  Emergency room usually does CT and treat the nausea and dizziness (meclizine).  Since the episode last evening i have been extremely tired, sleeping most of the day.  I have noticed that since this has been happening that i become startled at the simpliest thing.  
Not sure if this has anything to do with the dizziness but in the last several months i have noticed that when i bend down i hear my back cracking and have had alot of pain in my lower back and left leg to the point that it keeps me awake. I did notice that when i pull my leg up under me (watching tv) that the dizziness seems to start.
I have had two previous MI and a stroke, so you can imagine that this is truely causing me a lot of stress wondering what is going on.  
Would appreciate any feedback.

Hi!
It is difficult to diagnose dizziness on net. But you have a previous history of stoke then a complete neurological examination should be done. At your age, even hormonal assay (estrogen, progesterone, FSH, LH should be done).
You need to get examined to rule out a few things such as low blood pressure, postural hypotension (fall in blood pressure on standing), cervical spinal nerve compression (this is possible because you also have lower back pain and possibility osteoarthritis of spinal joints), hypoglycemia (low blood sugar), severe anemia, an underactive thyroid (called hypothyroidism), overactive thyroid (called hyperthyroidism), adrenal insufficiency (called Addison's disease), GERD or gastric reflux, and internal ear infections.  Please discuss with your doctor.
