Unable to focus when reading, foggy, a sensation of being disconnected.

Over the past several years, I have been have some seriously weird symptoms with my eyes. These symptoms are very hard to explain because I have almost no idea how to describe most of them. My sight isn't that bad and I do wear glasses too. I have some kind of sensation around my eyes that I have no idea how to describe. It is not eye fatigue nor is it eye pressure. It does not cause pain nor does it ever go away.  It does however affects my ability to focus, especially when reading. I have double vision and blurred vision randomly, but only for a short time, even with corrective lenses. This sensation, if I let it, would have me feel disconnected.  When I try to have conversations, it is hard to focus on what we are discussing, harder to think of what to say, and even harder to remember what we were talking about afterwards. It's almost if I have a foggy mind 24/7, just worse some times than others.  This sensation is what I feel through all of what I just said, I feel it mentally and visually.  I do tend to get frontal headaches frequently which last for several hours, but I usually think it is a sinus headache. I just mention the headache in case it is important. I do not even remember exactly when all this started.  I am only 16, my parents either do not believe me or doesn't think that it's a big deal.  It's hard to tell them when I do not even know what it feels like.
I have been trying to do some research on what it might be and I came up cold.  The only thing that I found even related described them having neck pain and pain between the shoulder blades.  Yes, I have had terrible neck pain and pain between shoulders but I have never thought to relate that to what I am feeling. Plus, the person who wrote the question (and the people who replied) all are a lot older than me and they say it had only lasted for the past few years. Does anyone else have this problem? Does anyone know what this might be? I have gotten tired of it and I really want it to stop.

PS: I might not have described all my symptoms, but those were the ones I can think of.

Have you gotten blood work to check your Vitamin D or RBC Magnesium?
