Whether or not to have ACDF

Due to a freak accident, I had to have an MRI which revealed that I have spinal cord distortion and stenosis at C4-5 and some narrowing at C5-6.  The only symptoms I have are some tingling in my fingers and on occasion some neck pain, very minor, that goes away with Advil.  Both the neurologist and neurosurgeon say I need an ACDF as I could be paralyzed if I trip and fall or get in an accident.  Because I have such minimal symptoms and lead a very active life (did 5 hours gardening yesterday) I am reluctant to have the surgery until more symptoms appear.  Also, there is the fact that more problems can occur after the surgery.  What do you suggest?

Thank you, Dr. Sharma.  I have an appointment to see the neurosurgeon again.  Will ask lots of questions.

Hi there. The issues of concern are stenosis of the central cervical spine may result in myelopathy from cord compression.  Treatment may be conservative or surgical. Conservative options include rest, physical therapy with strengthening exercises for paraspinal musculature, bracing, use of optimal postural biomechanics, NSAIDs, analgesics, antispasmodics. Surgical decompression is indicated for people with incapacitating pain, claudication, neurological deficit or myelopathy.  Stabilization is reserved for them who have segmental instability. Your neurosurgeon is the best judge. Hope this helps. Take care.
