Thank you for that comment. The diagnosis is arthritis. There is no dorsal osteophyte present. (And no gangrene!!)
What I am actually looking for are people who have had an operation for an arthritic big toe joint and what success/failure they have had.
There are a number of different operations that can be performed including the relatively new innovation of joint replacement.
We want to find an experienced podiatric surgeon, not a general orthopaedic surgeon with little specific experience.
Has anyone any recommendations?

It seems that the pain has been very intense. Before going for a surgery and taking the extreme decision, what has been the diagnosis? Go in for a surgery if there any symptoms of gangrene. Otherwise wait and watch and take ibuprofen for pain relief.
