Kicked out of Pain Clinic

I have been going to a pain management clinic for years.  Never had any issues.  The past month they switched my meds and put me on the lowest dose of a fentanyl patch, and I take 5mg vicoden.  Well the new patch wasn't working.  I called them for almost 2 weeks straight.  The nurse called me and said she would talk to the PA and get back to me.  I called more, and then I get a call (which I missed) so I got a voicemail at 5:50pm..  The voicemail said "we are walking out the door, but wanted to call you back.  Have a good memorial day weekend.  Call us on Tuesday"..  I called them Tuesday and left yet another message and no one called me back.  So seeing I was in so much pain and couldn't go anywhere else for medication (due to my contract with pain clinic) I used 2 patches..  So I resheduled my next appt for a week earlier then it was supposed to be to explain to them the issue.  I am very truthful about what I do..  So I told them exactly what I did seeing they never called me back.  She told me I was in trouble..  She told me I was 2 patches short.  Then she said they tried to call me a number of times ( which they didn't)..  She told me that I self medicate and she won't lose her license over me.  Then she wrote me a script for the same stuff (which doesn't work) and sent me on my way.  So as soon as I got home I called my doctor to speak with him, instead of the PA who won't see me anymore. I left a message for him or his nurse to call me back.  I am going to continue to call my doctor at the pain clinic until I talk to him.   Hopefully my doctor will let me back in seeing I have never had any issues before.  What exactly should I do?

ALL THEY CARE ABOUT IS LOSING THEIR LICENCE, INSTEAD OF DOING THEIR JOB CORRECTLY. IE I DGAF about you, you’re  licence, contract, or clinic. Now do your job like a professional - she’s saying “I only care about my wellbeing, my job/check and don’t care about or for my patients at all...

It’s this archaic everywhere it seems... ever since I looked into PAin

Pain management, clinic sounded perfect specialists and all, but when I heard of these evil, imho, contracts solely to cover the PMP and PM clinics ***, and make life as much as a pain as possible. Meanwhile junkies get a lifetime supply of methadone or suboxone, for maintenance... I’m at a loss for words, the addicts they get all the relief, hand over fist. Which I’m fine with I hope they’re all doing better, my friend (& cousin on subs) is on methadone (barely can tell) and I know they don’t count pills, drug test, nada. Just here ya go!

Now WHY should it be any different for us, non addicts, that need pain management with clean wrap sheets, no history of abuse etc etc. Why are we singled out? Also, why the Orwellian tone and approach? Eventually people will leave due to lack of actual care and harassment and then they’ll have no clients...and no $$. IMHO this seems like a violation of the Hippocratic Oath, they pledged to do no harm and to always ensure The Patient is comfortable... fml

I am sorry to hear of all the issues you had with this medication.  Sounds like you had a very hard time.  I have not really had any of those reactions as of yet, thank god.  I have very dry skin, and have to use moisturizer daily..  So they are hard to stick to my skin.  I put them on my *** and put medical tape around the edges.  I wear my patch in the tanning bed, in the hot bath, and in the pool.  Have not had any issues with it thus far.  Went back to my doctor today..  They have upped my meds yet again.  Now I am on 50mcg every 72 hours.  Plus my vicoden (which stayed the same)..  I talked to the doctor about which she thought was best, the gel or hard patches.  She said that was up to me.  She told me to try all the different kinds the pharmacy offers and when I find one I like stick to that and make sure they order that every month..  I know you had a bad time with them, but if you have any advice of which patches are best I would love to hear your opinion.  I have only had the hard patches thus far, and am going to switch to the gel patches this time if my pharmacy offers them.  I was told the gel patches have a completely different adhesive then the hard patches.  So wanna try those and see if they stick better then the ones I have.  With my skin issues I have heard that the gel ones will work better.  Just hope that my pharmacy carries what I am looking for.  My doctor said it was up to me to find which ones work best, and then stick with those..  So I guess that is what I will do..  I have tried 2 different hard patches now, and one worked better then the other.  So now I guess I will try and see if the gel patches work better then the hard.  Like I said I know you had a terrible time but any advice you can give me is greatly appreciated..  Thanks again..

plus since I had a patch on. Definitely never going to be bikini time for me ever again in life. That's just a minor thing though, meaningless.

There are much more profound problems encountered with long-term fentanyl use. The gel patches are actually "sharp" on the edges and may hurt or cut you - they did me. They all hurt me. I promise.

I have known many folks who have used these patches - all available kinds. None of them currently use them. Not one. 2 people I know very, very well attempted suicide. Both were relatives.

One succeeded.

She did this after her medication was cut off due to an incident close to what you described - very close. It was her second problem. She called the nurse too many times, according to that office.

The withdrawal from a higher dose of fentanyl is worse than anything one can imagine. It is absolute hell. Your dose may likely increase, as mine ald others I know well did over the years. I went from 25 mcg q 72 hrs to 200 mcg q 48 hrs over 13 years, and fought it every step of the way, oozing and bleeding through every piece of clothing I owned, in square shaped stains, no less. Finally, the patches just simply fell off after my allergy progressed, after a day or less, resulting in my going through withdrawal every 6 or 8 days, every month. The PA knew this, had photos and viewed the painful scabs and current sores every month as well. My situation was well-documented, yet it was me who finally had no choice but to handle it.

I have never, ever, encountered anything like what I went through when I refused to take that medication any more, and my current physician - in another state - has expressed disdain at such conduct by those medical professionals. My doctor then was very angry with me, yelling at me for my "stupidity" for all to hear in that office. Very, very angry. He actually discharged me soon after for trumped-up non-compliance, and again, I showed him the gooey dripping bleeding pus beneath every single patch every few times I actually saw him.

He had nothing to say, other than I was obviously allergic to the adhesive, prescribing antibiotics every time. This was ultimately my reaction to every single brand and type - gel and flat - including the gel name brand, Duragesic. My reaction, not everyone's reaction.

Please, please be careful. I am not implying you would have the same reaction - I'm only sharing personal experience as one who endured initial reactions quite similar to yours. I am in no way advising you on a course of action, merely sharing my own experiences in this forum.

PLEASE, also know they (have literally) floated off my body in a bath, and no matter what, if they contact water for longer than 30 seconds, you could likely experience either drop off, or you could feel symptoms of overdose, as heat, perspiration and activity causes the patches to release more medication. This fact is well documented.

Once again, these descriptions only express my personal experiences and opinions, and in no way should be construed as indicative of everyones experiences with this medication or advice as to the use or decision(s) involved in others use of the medication fentanyl. This statement applies to this, my current  post, as well as the previous one of this date, June 30, 2014.

Fentanyl user for 13 years here. The weeping sores will never subside; I know this from personal experience. This will get worse, wait and see.

Gel patches get clogged up - the tiny holes get blocked by skin cells, plus, the oozing sores also block the medicine contact with your skin.

As far as nothing happening? I promise, it did. You did not comply - non-compliance, to be exact. Something will happen to you there, at this point it is CYA time for them. Their records clearly do not reflect adequate response to your well-documented problems.

I would recommend your searching for another pain management physician, and soon, before you are discharged for some trumped uo reason. I'm sure you are happy happy right now, but your immediate problems with the patches are bound to cause you much more profound difficulties.

There are other long-acting medications that can be utilized, and I'd heartily recommend your asking about them. Patches fall off for no reason, float away during a bath, cause long term scarring (I'm still scarred after allthose years and it's been 3 years [

The WORST treatment I ever received was there, and I had a broken back 15 years ago with now titanium from my waist down, and so many problems I cannot go into them all. 36 total operations, not including the procedures. I moved because of the incredibly poor care. I lived in Lexington. I would never ever move back home.

There ARE NO GOOD PAIN DOCTORS THERE. PERIOD.

You should try out of state. There is really, unfortunately, no other option that will work for you - or anyone. I am so sorry you are going through this.

My suspicion is that particular doctor may have some authority jumping down his neck for an entirely different patient who may have reported him.

I don't know anything about fentanyl patches, hopefully someone on here can help you with that. Glad you are back with your pm dr, only people with chronic pain know how important they are to us to live some sort of normal life. Hope you get the right does of meds to get the relief you deserve. So I went back to my pain management clinic of monday!  I saw a different physicians assistant..  One I really like.  She was great.  She changed my fentanyl patch to a 25mcg patch and upped my vicoden for breakaway pain to 4 a day.  So I took my script to the pharmacy.  They had to order the fentanyl patch and it should be in today.  I have tried the mylan and sandoz hard patches.  I am having a bit of a reaction (they think it is a reaction to the adhesive)..  The doc wants me to try a gel patch but the pharmacist didn't really understand..  So god knows what I will get when I pick the script up today.  He said he will order the sandoz but no clue what kind of patch it will be.  I have no clue being new to this medication so we will see if I have a reaction to this one.  If you have any input on this let me know.  I don't know much about the fentanyl patches.  This is new to me..  But I am back in at the pain clinic like nothing happened..  Thank god.  Thank you all for your comments and concerns..

have you ever been diagnosed as lactose intolerant???  i ask because suddenly at the age of 15 i started having bad stomach issues.  had the same tests as you and the ulcers were caused from the lactose.  it is in so many things, even popcorn.   you may want to stay on a lactose free diet and see if it gets better.  if that is the only thing getting you treated for you pain that is crazy.  i had to drink a HUGE thing of straight lactose in the hospital and get blood drawn every half hour as well as the scope things.  all for lactose intolerance.  i had no idea it could pop up out of nowhere.  when those ulcers are there it keeps you from absorbing most nutrients as well.  hope you get some relief soon.  so kentucky opted out of the affordable health care act????  that is too bad if they did.  people should not go bankrupt or without care.

So I finally got some good news.  I have been calling the doctor or his nurse since the 30th..  I have called 3 to 4 times per day, everyday, and left messages for one of them to call me back.  Finally this past Friday the head nurse called me.  I explained to her what happened..  How I used 2 patches before they were to be used.  How the PA didn't listen to me..  How I didn't find this fair..  How I came in and told the truth..  How I tried to call them for 2 weeks with no return calls, etc.  She said she would talk to the actual doctor and call me back.  A half hour later she returned my call.  She told me she spoke with the regional manager.  She spoke with all the nurses and everyone I have seen in the past.  She read all of my urine screens for the past 4 years I have been going there.  I have never had a bad urine test..  The people there said I am a model patient.  No one had anything bad to say about me.  The regional manager said I should not have been kicked out.  So I have been reinstated into the pain clinic.  It has been put in my chart that I will no longer see the PA that tried to kick me out.  I have an appt this monday to have the meds adjusted seeing the PA gave me a script for the same **** that didn't work and sent me on my way..  So I finally am good to go and can get my meds adjusted to where I can live normally with minimal pain!  Thank you so much for your advice in this matter.  Wanted to let you know it turned out good and in my favor..  Thank god!

I ASKED IF I COULD SEE ANOTHER DR THERE AND THE NURSE SAID NO. RIGHT NOW TRYING TO CATCH UP ON MEDICAL BILL HAD GALLBLADDER CANCER SO I HAVE TO SEE AN ONCOLOGIST AND THAT IS 100 DOLLARS A VISIT AND AFTER THAT HAD TO HAVE MAJOR SURGERY TO GET BIOPSY OF LIVE AND SOME LIMP-NODES., HAVE NOT BEEN THE SAME SINCE MY STOMACH IS SO BLOATED LOOKS LIKE I AM READY TO HAVE A BABY. AND KEEP GETTING SHARP STABBING PAINS THROUGHOUT MY STOMACH, LIKE SOME ONE IS STABLING ME WITH A KNIFE, SENT ME TO GET 3 CT SCANS IN THE LAST 2 MONTHS KEEPS GIVING DIFFERENT DIAGNOSIS, JUST SENT ME TO A GASTROENTEROLOGIST , HAD TO PAY HER OFFICE CALL PLUS SHE DID A COLONOSICPY AND ALSO WENT DOWN MY THROAT. SHE SAID I HAVE ULCERS IN COLON AND DIGESTIVE TRACK, HAVE BEEN TAKING MEDS FOR THAT FOR A WEEK AND A HALF, AND STILL IN A LOT OF PAIN  . SUPPOSE TO GO BACK TO ONCOLOGIST ,JULY 1ST, BUT ME AND MY HUSBAN AGEE , WE HAVE BEEN GOING TO HIM FOR A YEAR NOW AND HE JUST WANT TO RUN MORE AND MORE TEST, ALSO HAVE TO GO BACK TO THE GASTROENTEROLOGIST JUNE27 , HAVE TOLD BOTH I AM IN TERRIBLE PAIN, BUT THEY WILL NOT GIVE ME ANY PAIN MEDICATION. TRIED CONTACTING KY BOARD OF HEALTH AND THEY SENT ME SOMETHING ABOUT CARING FOR THE ELDERLY. IF ANY ONE KNOWS A DR. have you tried contacting them for any reason since that happened???  maybe his wife died, fell ill or just left him for another man?? maybe he feels bad about his behavior???  just a thought.  he sounds mental, but he is human and may have been going through something really horrible.  no matter what, i would never see him again, but maybe you could see another doctor in the clinic??

one thing I would do for sure  is file a complaint against that doctors with the medical board.  first try and find out the OWNER of the pain management clinic and contact them as well.  you have witnesses.  you have a great case imo.  also get a copy of your records and see in your contract if it say's anywhere 'you are not to mention my wife' and if it doesn't consider contacting an attorney.  some of these doctors think they are God and they can control your life by controlling or not controlling your pain.  it makes me sick.  i have been to paducah a few times and it is a pretty small town.  you can drive over the bridge and be seen in illinois.  have you considered that??

have you  considered calling a malpractice attorney for a free consultation???  your medical records BELONG TO YOU and they are supposed to give them to you.  i do believe they have a turnaround time of about 48 hrs (different everywhere) and if that time has passed, I would physically walk into that office and say 'i am here to pick up my medical records.  i called and requested them on such and such day and time and i am here to pick them up'.  also make sure in those records is your SIGNED contract.  that is one of the most important things. read it over and if it does say 3 strikes and you are out and your records only indicate the one time, take that to an attorney.  who knows, when you go in to pick up your records and after they hand them to you, mention what you are doing with them..... talking to an attorney about patient abandonment or them breaking the contract and they may just call you back

Have you tried going by the office, they may not like it, but you have played by their rules, and they won't call you back. I have had to do that before cause of a misunderstanding, I had a bad flare up and the nurse called me back, and changed how many times I was suppose to take my meds, until they could get me a procedure done, well when my early refill came, they had no notes in my chart, so I went down, and the nurse remembered that, luckily. When you call, the operators know nothing about you, and you also never know who is calling you back. I feel your pain, been there before I went into pain management. I hope you get the help and answers you need. It's a shame that you are being punished for telling the truth.

The bad part about the pain clinics here in PA..  They make you sign a contract.  Stating that they are the only people that can prescribe any kind of pain meds for you..  That is where I got into trouble.  I couldn't go anywhere else and the pain clinic wouldn't call me back.  I have been calling my pain management doctor since the 30th, and still haven't heard from him or his nurse.  So I call them constantly.  In between 2 and 5 times a day..  So they are eventually gonna get annoyed with all the messages and call me back.  I have also left two messages with medical records cuz I want a copy of my records and my contract.  But no one will call me back there either..  So that is great!  I hope that you find a solution.  I know how terrible it is to be in pain.  There are all these people that abuse drugs, and those who use them the way they are supposed to be used get treated like crap.  Figure that one out!

Do you have a family dr or surgeon? They might could give you some meds to help until you get an appointment, and they can help with that too. I think we all pay for the drug dealers and abusers, the laws seem to only hurt the people who are in real pain, the dealers still gets there meds. Just like here in Alabama you can't even get lortabs anymore it's changed to norco, with 325, instead of 500, of acetaminophen, the med in Tylenol, because the medicine was being abused so much, well that don't stop people from abusing it, but to me it doesn't help the pain as well, so it only hurts people with real pain. Hoping you get the help you need.

I THINK THEY ARE DRUG DEALERS ON THE SIDE ONCE SOMEONE CAME UP TO MY HUSBAND, WHILE HE WAS WAITING IN THE CAR AND  AND ASKED HIM IF HE HAD ANY LORA TABS(NOT SURE ON SPELLING), BUT , MY HUSBAND IS SEVENTY YEARS OLD AND OFFERED HIM SOME EXCEDERINE, AND THE KID JUST LAUGHED AT HIM. YOU CAN WATCH THE PEOPLE COMING IN AND OUT AND YOU CAN TELL WHO IS THERE FOR HELP AND WHO IS THERE JUST TO GET DRUGS. I REALLY THINK THE ST. OF KENTUCKY OUGHT TO TAKE A GOOD LOOK THEIR RECORDS. I JUST WISH I KNEW SOME DR. OR ANOTHER PAIN MANAGEMENT OTHER THAN THE PADUCHA PAIN MANAGEMENT. MY HUSBAND WOULD KILL ME IF I EVER WENT BACK THERE . DR. MALLA IS A DEALER NOT A HEALER. IF ANY ONE KNOWS OF A DR. OR PLACE THAT WILL TAKE ME ON I WOULD BE EVER SO HAPPY TO TURN MY RECORDS AND CONTRACT OVER TO THEM.

thank for responding . i did sign contract, but i did nothing wrong. kept every appointment. never over used medication. paid every time i went. Dr. malla a nut case if you ask me went nuts , because my husband knew how much pain i was in and got mad because Dr. malla called me lazy. So mu husband went to next visit with me. and after talking to the Dr. and he finished writing prescriptions , all my husband said sitting in a chair., was what if it was your wife, he went crazy in the hall outside of my room , screaming don't say anything about my wife and continued to scream that i was not going to be seen there anymore, and he never wanted to see me in the office ever again. He even scared the nurses, he gave me 1 refill,and when she came out , could tell she was very nervous . i asked her if I could see another Dr. there she quickly said no and left. i did not say one single word to the Dr. and am positive my records will show this to be true. this all happened in 2013. and have been in constant pain ever since. i don't understand why you have to go to pain management to get help with pain. i use to live in Michigan. before we moved here 7 years ago . THE PRACTICE OF MEDICINE AND BEING A DR. ARE WAY BEHIND. SICK OF HURTING

I did sign a pain management contract years ago when I started going there.  I couldn't even tell you what it says at this point.  I have been going there so long with no issues what so ever, I don't even remember what all the conditions are.  I do know that the first doctor I saw when I started there told me about the 3 strikes and your out rule.  Well this would be my strike one!  I do understand that I messed up by using the 2 patches before I was supposed to..  I will own up to screwing up..  But at that moment of severe pain, when I can't go anywhere else, and no one is calling me back, I did what I thought would help me..  Seeing no one else seemed concerned about me ya know?  I have a log of everytime I have called them.  I am keeping track of it..  That way when I do talk to the doctor I can give him the list of all the times I called, and the 2 times they called me...  The first time they called it was the nurse, she said she would talk to the doc and call me back.  The second time they called they said we are walking out the door call us tuesday, have a good weekend.  That message really pissed me off..  Have a good weekend my ***.  It was like they were screwing me on purpose...  I am just so frustrated.  I have never had any issues in the past.  I have been a model patient.  Then I screw up one time and the physicians assistant kicked me out..  I have talked to people in the waiting room that tell me they got caught for illegal drugs in their system on their **** test, and they didn't get kicked out..  WTF is that?  And I get kicked out for this?  The physicians assistant said that I self medicated, because I used the 2 patches before they were supposed to be used, without permission.  So that is self medicating.  I consider self medicating when you take something you are not prescribed..  Maybe that is just me..  I am just in shock over this..  I can't believe I have been going there for years with no issues and then one screw up, because no one would call me back to help me, and I am out..  Does that make sense to you?  I mean am I looking at this the wrong way or do you agree with what I am saying?  I will continue to call this doctor everyday and leave messages, and keep a log of when I call, until I talk to someone!
