mri report

I have had two shoulder surgery over the last 14 months. I am still having great pain when rotating arm up and outwards and loss of strength. orthopedic surgeon ordered mri. MRI showed increased signal involves the supraspinatus tendon and infraspinatus tendon with focal signal abnormality of the distal distal conjonted tendon suggesting partial to near complete tear. aditional endinopathy is present involving he conjoined and distalsuraspinatus tendon. fluid is present involving region of the ac joint suggestig surgery and along the subdeltoid fat plane. mild fluid signal is resent involving subscrpularis tendon insertion sugesting tedinopathy.

does this indicate another shoulder surgery is indicated. i have had steroid injections and been in therapy with no relief,

had surgery today doctor said it was worst then he expected, he found complete rotator cuff tear he also found laburn tear and my bicep was torn looks like a long road ahead of me. sorry about the typing trying to use one hand not for sure on the procedure he used for bicep i think it was bicep tenodesis but not for sure..

I also forgot to tell you that my surgeon also has a great reputation as well, but does the same as your guy, shoulders and knees.

I have had my case reviewed by two other surgeons that work on the shoulder exclusively and they have both told me that they would have addressed my issues differently from the beginning. The one thing about Orthopedic Surgeons is they do the surgery and then you are sometimes left in more pain then you were in before you had the surgery and then they don't want to give you the medication you need to cope with it.

My PCP takes care of my pain management. If it were not for him I would be in a real jam. It is very difficult to do simple tasks that I used to take for granted since these shoulder surgeries. Once the open procedures start it gets worse. I hope you don't have to get to that point. The problem is, once we get past the age of 40 it is very difficult to bounce back from this type of injury.

Thanks on the retirement. I left 18 months early because of an ankle injury but must shoulders would have done it to me as well.

It sounds like your doctor took the conservative route as mine did in the first couple of procedures as well. The issue with the collar bone seems to be a recurring factor for you as it was for one of my partners. He had the same exact problem as you are experiencing and had 4 procedures.

Because you are continuing to have pain and loss of range you are most likely facing another procedure even if it is exploratory surgery. The fact that you are experiencing weakness even with PT indicates that the tear that you have is more serious than you doctor thinks it is.

I am on my way to the operating room for a 5th repair of the Infraspinatus and the Glenoid Labrum on my right shoulder. My issue is unique as well as it is all posterior which only occurs in a small percentage of shoulder patients. I think I may have messed them up weight lifting with a wide grip on the bar while bench pressing dating back to my teenage years. Who knew?

Thanks for your reply. I am suspecting you are right about going back to work to soon kind of a combination between the insurance company and myself. To answer your questions.

1. My first diagnosis was RCI.

2. My surgeon removed some of the shoulder blade and shaved underneath the colar bone to give more room to the ac joint. However, I did not have any maajor tear to the rotato cuff.

3.It was arthroscopic surgery.

4.I started on therapy at home the next day in the chair to get back rom. Then after a couple of weeks, I started going to therapy 3x week for about two months. I was doing real good until about 5 months in it and started having pain. My surgeon performed an x-ray which showed the colar bone had started to grow back, which is very unusual. He then performed the second surgery and found the bone had grown back and had torn the rotator cuff. I then did the chair and started back to therapy for about two months again. I was returned back to full duty around 3 months after th second surgery.  I then got into an altercation with a DUI driver and started having pain in the shoulder when i raise and rotate my arm outwards, loss of strength an a lot of motion.

5.My surgeon is a shoulder and knee specialist with over 20 yrs exp. He is known to be one of the top shoulder doctors in my city.

6. I have never had a shoulder dislocation.

My surgeon reviewed the mri film and stated there was fluid in and around the ac joint and a tear. However, he did not think it was a full tear. However, the radiologist report stated he believed it was almost a full tear. My doctor said he wanted to try injections and therapy. So we did injection and I have been in therapy. However, I have received no relief of pain or rom. I go back to doc in a week. It looked like the mri showed th anchor and the tear that was fixed is still intact but the tear had moved to another location. My surgeon said on the last visit he would probably go back in and look around if i did not get better.

It is real nerve racking and a little scary thinking about your career and if you will be able to continue with the injury are if they can fix it and be able to continue to stay in the line of business we r in.

ps  congradts on your retirement and hope your shoulder gets to doing better.

I am a retired police officer (just retired on 12/1/09 after 23 years) and I have had (8) shoulder surgeries in the past three years so I am your guy!

Your biggest problem right now is the tear and where it is located. The more pain and weakness you are experiencing the worse off it is going to get. I am suspecting you returned back to full duty too soon and this caused your problem. Been there, done that so don't beat yourself up over it.

Please answer some questions for me so I can share some more of my experience with this with you.

1. What was your initial diagnosis?

2. What type of surgery did you have? Was it a rotator cuff repair of a tear, or a shoulder reconstruction?

3. Was it open surgery or arthroscopic surgery?

4. How long were you in physical therapy before returning to full duty?

5. Does your surgeon specialize in shoulder surgery?

6. Do you now or have you ever had a shoulder dislocation?

I will tell you this now. When it comes to the shoulder your chances decrease by about 30% to regain full range or even 70% after more than one surgery, especially with what we do for a living. I ruined the surgery in my right shoulder just throwing my equipment bag in the trunk of the radio car after the first surgery.

I don't want to alarm you that is why I asked you all of these questions. There are several different things that can go wrong in the shoulder as it is the most complex joint in the body. We all heal differently as well.

thanks for your input, I am a police officer and am having quite a time getting over this shoulder injury and performing my job functions. My surgeon look at the mri and said he did not know or think the tendon was torn that bad. Dont know why he thinks its not and the radiologist report says something different. I been in therapy with no results.

Welcome to the Pain Mangement Forum. I am glad that you found us and took the time to post. Understand that I am not a physician so my response should not be considered an expert opinion.

After two surgeries I would have hoped that your shoulder was repaired. If you still have a near complete tear I would think that at least a laparoscopic surgery would be indicated. Your shoulder has to be functional for a long time to come and as pain free a possible. I might consider changing surgeons or at the least obtain a second opinion.

You have certainly been through much over the last 14 months. You are very young to have such an involved shoulder condition. You must be in a lot of pain. I am sorry for that fact.

We have a member that has had numerous shoulder surgeries and was slated for another. I will also try to contact him for his input.
