Screaming 10 yr old

I have a 10 1/2 yr old son who gets very good grades at school and is very respectful to his teachers and to most adults.  However, when he is at home he loses control very easily.  When things don't go his way he screams, cries, throws things, slams doors, etc.  He also does this if there is something he doesn't want to do.  Ex.  He says he wants to play baseball and has for several years but doesn't want to go to practice and will throw a screaming fit to get out of going.  

This same child also has another issue.  He still poops his pants and will not get his bottom clean.  He can go to the bathroom and poop sometimes and will make a passing swipe and cleaning his bottom but then off he goes and the smell follows.  Other times if he is out playing he won't even bother to come in and do his business and just go in his pants and live with it until someone says something to him.  I am wondering if there is a medical issue with the pooping of his pants and if I need to seek out an adolescent counselor for his temper.

What triggers the 'tantrum' what has happened immediately prior to it ? As he is okay at school it does sound if the problem lies at home ,when you say things 'dont go his way ' can you enlarge on what things ?
