Bladder shut down after repair

I had a hysterectomy and bladder repair last Thur. It went well, they left the ovaries and they did it all vaginally. They were able to do the bladder and urethral repair without the mesh....,OK they left a catheter in for the first 24 hours. When they took it out, I was able to go a litle intially but that amount decreased throught the day. I was sent home with a in dwelling catheter. Today, Sun as instructed I took i out, then tried to go. I cant go at all... I keep trying and self cathing every 2 hours. I just can't go. When I cath there is never more the 200cc. I am trying to retrain the bladder so self cathing instead of putting
the indwelling catheter back in. I think the catheter made it worse so I am not putting it in. ???  ANY INPUT OR ADVICE!

Thanks for sharing the info Shelly! Bladder shut down is common after effect of surgery. Everyone is a bit unique with body reaction to any  surgery but of course anything related to POP will impact bladder even more. So glad you got this vital info from your urologist quickly!

I did hear back from my urologist.  He gave me some good advice that I will share here to help someone else. He told me that the bladder shutting down post op is normal even in healthy bladders which mine is not.  He basically said what you did that I needed more time. He told me not increase fluids, or the frequency i self cath etc. He told me just to cath only as needed for comfort, and not 2 let the amounts n the bladder get
greater the 450 cc, However he said this is normal and it will return to normal in time!

I am so happy to hear things are leveling; I'm sure the steriods made a difference. Sounds like you are totally on top of everything Shelly! Just need a little more time. Be aware that as you feel better and start to move around more, swelling may come back up again for a bit-normal.

Actually, I don't have to go to the ER they sent me home with extras and I am trained to both put one in and take one out.. Tonight I was actually able to urinate at least 1/2 of the urine out..., so seeems to be working.  I had them give me ice post op and lost of anti-inflammatories, so honestly not much swelling. They also did a bladder instill post op and then did one later when I was in a flare. The instills not only contain lidocaine and marcaine but also solu cortef... which is a steriod. I am sure that has helped with the swelling.  I do have an e-mail i sent to my dcotor as to what I need to do for sure.  I also have Intestial cystitis which is why they did the instills etc. which is also what complicated things. Actually they did an epidural for surgery to help with the IC post op.too. When they did the surgery I felt nothing at all, until they started on the  bladder and bladder repair, then I could feel ever thing.., just hoping we can get it to working now

Ask them to put the cath back in (may have to go to ER) and leave it in for a couple more days rather than self-cath. Sometimes the swellling takes a bit to go down and until it does urinating will be difficult. It is common to leave a cath in for 5 days or so after POP surgery (I don't know why they even take it out to begin with, unnecessary torture). I think it would be better to give your bladder a break for a bit longer, once swelling has leveled you can shift forward retraining it. the self cath is probably irritating already inflamed tissues. I would be more worried about injury to bladder and kidneys by retained urine than the retraining at this point.

Please check with your dr on this Shell-please don't wait.

Also are you doing anything externally to reduce swelling? Ice is so important the 1st week after surgery (I put ice in preemie baby disposable diapers, any icepad or frozen corn will do). After the 1st week switch to heat, if you have a micro bean bag that works well because is can be bunched into the crotch area nicely. I used this for a few weeks after POP surgery.

Please let me know how things settle down for you as you can. You are in my thougths and prayers hon!
Sher

Hope things level for you soon.
