It's usually found at routine appointments and/or ultrasound. You might sometimes have a decrease in symptoms, but many don't and it's not known by the mother. In some cases the body never expells the tissue and she has to go in for procedures such as a D&C or take medications to force the body to expell it.
ic .. so the mother would never know that the baby is gone .. ??

Not necessarily. It's possible to have what's called a missed miscarriage at any stage (until 20+ weeks and then it's considered a stillbirth) and in it your body doesn't realize the baby has passed and doesn't do anything to expell the tissue.
