tell your friend to try using a natural soap or one that is for sensitive skin. Soaps that are full of chemicals can irritate the sensitive skin on your penis and scrotum, irritation leads to dry and/or itchy skin; dead skin clogs the hair follicles and spots can form.

The spots can also be caused by sweat that isn't bening cleaned off properly. I'm not saying your friend doesn't shower. The sweat could build up after sports and if it's a few hours before he takes a shower thats enough for bacteria to to start causing spots.

If he plays a lot of sport, taking a shower soon after could help reduce the spots.

DON'T BE TEMPTED...Tell your mate to avoid using acne treatments on his penis. These are often too concentrated and will irritate more then they will help!

Yes, spots down there can get pretty big sometimes!

Yes, it probably is acne.  Hair follicles can get blocked, which will cause sebaceous fluid to build up and a spot occurs.  You can squeeze them (like acne on your face or wherever), but it's quite tender down there!
