How can TSH high level 5.510 and have normal range t3 - 28 -t48.4 Free thyroxine index 2.4

Please post values with given reference range as it vary from lab to lab.

Based on such TSH value it is not required to start on thyroid hormone replacement pills. Anyway even if start the value required is very less possibly less than 20 mcg levothyroxine or 1/4 of 1 grain NDT.

Also its worth to have a look on your sleep patterns, diet especially sugary foods etc as I think you might be experiencing stress related issues straining your adrenal glands.

Please do these blood tests to rule out deficiencies in Vitamin D (25-hydroxy D3), Ferritin, Vitamin B12

Before discussing those lab results, please post the reference ranges shown on the lab report for those tests, and confirm those are Total T3 and Total T4.  Even more important,  please tell us about any symptoms you have.
