High TSH with negative antibodies

My husband has a TSH of 4.8 Free T4 of .89 and tested negative for Hashimotos.
One doctor put him on Synthroid, but his main doctor took him off based on the negative antibody tests.

He has severe insomnia, anxiety, muscle aches, etc.and has been on many medications such as Zoloft, Cymbalta, trazadone, etc. and a variety of sleep meds without relief.

Can a person have hypothyroidism without Hashimotos?
Is the elevated TSH caused by all the different medicines he was given?

The doctor who told him no thyroid meds unless he was positive for Hashi's is an idiot...sorry, but although Hashis is a very prevalent cause of hypo, it's not the only one.  Iodine deficiency is another, but there are many.

Your husband's FT4 is only 7% of range.  The guideline is 50%.  He is clearly hypo.  His thyroid is not functioning properly.  

Which other meds is he on?  Some meds can cause thyroid deficiencies.

Hashi's is just one cause of hypothyroidism.  Which antibodies were tested?  In order to rule out Hashi's, both TPOab and TGab have to be negative.

His FT4 looks low, but if you could provide a reference range, that would be helpful.  Did they test FT3?

SSRIs can affect thyroid hormones.

How much Synthroid was he on?  For how long?  Did it help with his symptoms?

He was on 25mcg Synthroid to start and was on them about a month before the other doctor took him off. The first doctor had changed a lot of his other meds so he was not really able to tell if anything was helping. The second doctor told him NO thyroid meds unless he was positive for Hashi's.
