Results of Biopsy are in....

Hello,

I had a biopsy of my thyroid nodules recently.  Results are as follows:

1st Nodule:
There is colloid, mixed populations of benign, monolayered follicular cell clusters, Hurthle cell clusters, and mild lymphocytosis in the background.  No evidence of classic papillary thyroid carcinoma is seen.  There is no evidence of intranuclear inclusions or nuclear grooves in these follicular cell clusters.  Some of the follicular cell clusters retain honeycomb appearance.

2nd Nodule:
The specimen is non-diagnostic due to inadequate colloid and follicular cell clusters.  There are very rare isolated Hurthle cells and very rare, one, cluster of benign follicular cells with small nuclei and abundant cytoplasm.  Mild lymphocytosis is noted.  No definite evidence of papillary carcinoma id identifiled.

My Endo said that they want to watch the 2nd nodule and do an ultrasound again in four months to keep an eye on it.

I was looking up "Hurthle" cells though and they seem pretty concerning.

Should I be at all worried by the results of this biopsy?  Not sure what to think on this.

I am not on any thyroid meds.

Oh, I also had the following test done too:
MICROSOMAL AB  Results 1.6 (Range is 0.0-9.0)  Not sure what this test is for.....

I agree with you on that.  Very expensive protocol with little to no science to back it up...  I contacted my local chiro who does that protocol once, just to see if he was who I thought he was, and I must have gotten an email a day from him for months.  Illustrates the power of advertising, I guess.

HI Goolarra,

I just looked at that institute on-line and I think its a Chiro/naturepathic office.  I am not even going to go in because they'll want to run a bunch of expensive tests (leaky gut syndrome, etc...)  I think it would be a complete waste of time.  I am seeing my endo again in 4 months (have to have another ultrasound of my nodule) so I'll talk to her about it then.

Hi Goolarra,

No, I don't but I get messages on facebook occasionally from them for some reason (not sure how that happened) and I see ads in the paper for them.  I think I'll investigate a little further.  I will let you know though and thanks so much for your help!!!

Hearing that I might sleep better really excites me!  ha ha

Just keep in mind that the guideline for FT3 is upper half to upper third of range.  So, lower end of range for FT3 is even further off the mark than lower end FT4.

Fatigue is a hypo symptom for just about all of us.  In addition, inability to stay asleep is also a hypo symptom.  I think there's a very good chance that if you got your thyroid hormones to a good level, it not only wouldn't make your sleeping problems worse, it might actually improve them greatly.

"Dublin Thyroid Institute"...the name would imply they'd know what they're doing!  Do you know of anyone who has consulted with them?

Thank you and goolarra for your replies.

Goolara, I had my T3 tested and it was also in the low range (but in the normal range) a while back.  I've been to several different doctors (I am also being treated for Lyme disease) and they always test my thyroid and say "your numbers are normal".  

Most days I feel pretty good but days like today, I wake up feeling pretty good but then hit a wall about 10am....always the same time and super bad fatigue sets in.  I'm thinking this fatigue is more related to these thyroid numbers rather than the lyme disease (I'm Igenex positive on Lyme - not CDC).  

Anyway, so I'm not sure what to do....  There is a place here locally called the "Dublin Thyroid Institute".  I might go to them and see if they can help.

One thing I'm worried about is that IF I was to start on meds for low thyroid, is that going to affect my sleeping.  I have a hard time getting a good nights sleep and have to take Lunesta/Melatonin just to get to sleep and even then, I wake up throughout the night.  I a little worried that if I were to take something, it would make that problem worse...?  Will it?

I just want to add that your microsomal ab, also called TPOab (thyroid peroxidase antibodies) are elevated, which indicates Hashimoto's thyroiditis.  

Your FT4 is quite low in the range.  Based on symptom relief, many of us find that FT4 has to be close to 50% of range.  Yours is less than 10%.  Most of us would feel very hypo with those results.

Your doctor should also be ordering FT3, which is the active form of the thyroid hormones and correlates best with symptoms.  

How do you feel?  Do you have a lot of hypo symptoms?

I would recommend getting second opinion on the first nodule, but in general the SHEETS of Hurthle cells is the cause for concern.
the nodules have colloid in them so they rather hyperplastic than neoplastic (benign nodule vs tumor).
Loose Hurthle cells are associated with Hashimoto's thyroidis.

Oh...here are a few more results I just noticed on same paperwork:

TSH 1.82 (.27-4.2)
T4 FREE .99 (.93-1.7)

My endo says all normal.
