Should a Dr. order surgery when any Hurthle cells are present?

OK. So, I'm 29, female, and have several thyroid nodules. The largest of the nodules has grown from 2.1 cm to 2.5 cm in 5 months and is a "Hurthle cell adenoma." My T3 has been low for many months and my T4 has recently dropped to right below the "normal" level. However, my "Dr." (who is actually an intern Dr. in training through the University) has yet to prescribe any meds for my many symptoms; fatigue, excessive sleeping, weight gain, dry skin, cold extremities,anxiety, trouble with memory, trouble decifering between cold and wet on skin, and this visible goiter which makes it difficult to swallow vitamins. Instead, the "Dr." has decided to wait another 3 months for another ultrasound before he is sure he wants to send me to surgical. Also, my Vitamin D was recently tested and was 15.5 which is very low I think.
I don't have insurance and have dealt with what i feel is incompetence since March of this year. I'm running out of steam and don't know what to say to say or do at this point. I would love to see an Endocrinologist without an ego but I won't be able to afford an expensive surgery.
On the other hand, I feel like my "Dr." is putting me off until my one year is up-at which point i will have to reapply for admittance into this program for the uninsured.
I would love to hear all thoughts, wisdom, and suggestions from anyone who has time.

Thanks so much for your comment! It really helped me to realize I'm not completely crazy.
I am taking a vitamin D supplement, along with a handful of other things to try to keep my body functioning adequately.( Although my Dr. never said anything implying that I should.) Still, I feel like 10 weeks is a long time to wait before retesting my labs again. I actually think he just scheduled the follow up for 6 wks after he decided to return my call with my lab results (4 wks after labs were taken.)
Have you had any Dr. ask you to fast before Thyroid Testing and Vit D testing? My Dr. says it makes no difference but my results finally dropped below normal after trying this.

Seriously, I would be trying to find a doctor you can relate to. You sound hypo and you need some meds. I had a Hurthle cell adenoma and it was surgically removed. Most doctors err on the side of caution and remove them. Having difficulty swallowing is showing that the thryoid is enlarged and you need treatment.
  I do feel for you. I understand the insurance issue. But I do know that if you contact the hospital you might have surgery at, they often can refer you to some place that deals with cases like yours (if you have to have surgery I mean)
   Start taking a Vit D supplement. You will more than likely be low on calcium too. If you do have surgery, this is a big issue where often the calcium levels drop quite dramatically. Many thyroid patients have low Vit D and low calcium.
