TSH greater than 150 & TPO is 400

My daughter has a small inflammation in the neck region.  Doctor ordered for the blood work and found TSH > 150 and TPO is 400.  What does this mean and she asked us to schedule an appointment with endocrinologist.  My wife is on thyroid medicine for 15 years. I am really concerned as to why ped did not give a medicine and recommended endocrinologist?

I am trying to schedule an appointment with endo however I have scheduled an appointment with my daughter's previous pediatrician so that she can evaluate and put her on medication.

T3 uptake, total T4 and free T4 index are all considered obsolete tests, of very little value.  That your daughter's pedi ordered these indicates to me that he is out of his element when it comes to thyroid problems.  Perhaps that's why he referred you to an endo.

Your daughter does need to start treatment, but you might not want her pedi treating her if he isn't comfortable and confident doing so.  Managing thyroid meds, especially in children, is a bit of an art.  If you don't know what you're doing, you can do more harm than good.  

Do you have an appointment with the endo yet?  Perhaps the pedi could be of most help getting you a prompt appointment with a doctor who can treat her.  Her TSH is very high; she should get treatment as soon as possible.

My daughter is 8 years old.  Her t3 uptake was 26 ( 22 - 35 ), t4 total 2.2 ( 4.5 -12.0), free t4 index  0.6 (1.4 -3.8 ).

Elevated TPO antibodies indicates Hashimoto's thyroiditis.  This is an autoimmune disease and the most prevalent cause of hypo.  It also runs in families.  Have your wife's antibodies ever been tested?

TSH of 150 is very high, also indicating that she is hypo.  Further testing is in order, however, to confirm this.  Her doctor should test FREE T3 and FREE T4 now, along with a repeat of TSH.  FT3 and FT4 are the actual thyroid hormones and much more important than the pituitary hormone, TSH.

I don't know why your doctor didn't prescribe meds until your daughter can get in to see an endo.  Does she have an appointment with an endo?  When?
