Thyoid problem?

57 yo Pt has very low TSH, low T4 and very high T3 with decreased uptake in radioisotope thyroid scan and small gland. She takes ACEI and is obese. What is the diagnonis?

Thank you so much for your answer. Can it be Hashimoto thyroiditis?

sounds like thyroiditis.  hyper phase, followed by hypo phase.  with the t4 being low, the t3 may soon follow.  then you'll be clinically hypothyroid.  

however, right now you seem to be hyperthyroid with the elevated t3 and low tsh.  ride out the hyper phase, but closely monitor levels to make sure you don't go too hypo...you may need thyroid replacement.
